Certain medications have been identified as High risk medications at Disha Eye Hospital
All these medications are used in house in various departments only and are not
dispensed to the patients.

• Define ordering, storage & dispensing of high risk medications.


• Stocks of the high risk medications are maintained in the stores stock register
by the stores manger and directly supervised by the treating physician.
• Once the stocks are exhausted, an indent for purchase of the medication is
prepared and sanctioned by the MD / CMO/Consultants and all Doctors.
• On arrival of the stock the packaging is verified for company seal, soiling or
damage of the external packaging. Expiry date and batch number is
documented in the stock register.
• Procedure for administration of high risk medication.
• Whenever these medications are prescribed the nurse will take special note of
these.
• The dosage prescribed will be checked by the doctor.
• If the dosage is outside the normal range then the dosage will be confirmed
with the consultant before preparing / setting up the medication.
• The ampoules or vials of high risk medication will be placed separately verified
for:
• The pharmacological name & date of expiry.
• Concentration & amount of chemical in each ampoule/vial
• Total dose if more than one ampoule / vial is used.
