Dear Dr. Cross,

Re:Mr. Brett Collister, D.O.B:20 November,1970

I am writing to refer Mr Collister, a 44-year-old

factory foreman, whose features are suggestive of
type 2 diabetes mellitus. Your further assessment
and management regarding his condition will be
greatly appreciated.

Mr Collister visited me few times last year with

tonsillitis,suspected rotator cuff tear and
suspected osteoarthritis which were treated
accordingly. Moreover ,he was advised to lose
weight and do exercise regularly because of his
high BMI.
On 4.1.15, Mr. Collister visited me with complaints
of weakness,sore eyes and dizziness for the
previous three to four weeks. He also reported
that he did not change his lifestyle. On
examination, there were no significant changes.
Consequently,few blood tests were requested.

Unfortunately on today's review visit,he reported

that he had no improvement of his previous
symptoms along with complaints of vision
impairment. His blood reports showed increase of
random glucose(13.5), fasting glucose(7.4) ,
HbA 1c(8.5) and cholesterol levels. Please note, his
current BMI is 28.4.

In view of the above, my preliminary diagnosis is

type 2 diabetes mellitus. I would be grateful if you
take over his care for further evaluation and
management as you seem appropriate. If you
have any queries, do not hesitate to contact me.
