I SANDEEP KUMAR . with registered CSC ID as 151232430015 . (here in after referred to as

"VLE-INS") who is duly Registered by the CSC e-Governance Services India Ltd. through
Certificate No: CERT-9481 . and Registration No: 151232430015 to conduct insurance business
as a VLE-INS is willing to carry out the sale and service of insurance products from my centre.

I confirm that I will abide with all the terms and conditions which are enclosed with this letter,
further I will also abide with Insurance Regulatory and Development Authority Act 1999 and the
regulations/ guidelines /circulars /notification or any other directive issued by the IRDAI and CSC
from time to time.

VLE-INS Terms & Conditions

1. GENERAL
Subject to the issuance, continuation of renewal of the license granted by CSC e-Governance Services
India Ltd. to the VLE, the VLE shall effective the date of issuance of the license by the CSC SPV ("Effective
Date") act as Village Level Entrepreneur - Insurance and Solicit and Service the Company’s insurance
products with which CSC SPV has an agreement.
The VLE-INS shall, at all times during this engagement be responsible to, comply and adhere with all
applicable:
Laws, rules, regulations, codes and guidelines of government authorities and more particularly with the
issuance act, the insurance rules, regulation, guidelines, notifications and directives issued there under.
The VLE-INS confirms having read the policies and understands that they form an integral part of these
terms and conditions. Further, the parties agree that in the event of any discrepancy in the interpretation of
the polices these terms and conditions, the policies shall take precedence.
The VLE-INS shall have the following responsibilities:.
1. To solicit proposals of insurance for the Insurance Company.
2. To meet the Standards indicated by the Insurance Company and CSC SPV from time to time on its
portal.
3. Maintain and continue to maintain necessary and adequate infrastructure.
4. Be fully responsible and accountable for all acts and omissions of self.
5. Bear all expenses incurred in the performance of duties as VLE-INS.
6. To keep his/her insurance license valid at all times and to promptly renew the license within the
stipulated timelines.
7. To not publish, cause to be published any advertisement concerning acting as VLE-INS, its business
in respect of which the VLE-INS is authorized to solicit or service in any newspaper, magazine,
publication or other media whatsoever, without the CSC SPV’s prior written approval.
8. To not make any misrepresentation or incomplete or inaccurate representation or comparison for the
purpose of inducing a person to convert, lapse, forfeit or surrender his/her insurance or terminate
his/her business relationship with any Insurance Company.
9. To promptly disclose to the Insurance company every fact and circumstance with his/her knowledge
relevant to the acceptance of the risk or business by the Insurance company and promptly and
accurately relate to the Insurance Company every fact disclosed to him/her relevant to the acceptance
of such risk or business.
VLE-INS Signature:
 1. To not act so as to put the Insurance Company at risk by delivering any policy, endorsement or
supplementary contract issued to a customer whose health or occupation he/she knows or has reason
to suspect has changed since the date of proposal.
2. Explain all the essential provisions of the cover offered by the policy recommended by him so that, as
far as possible, the prospective client understands what is being purchased.
3. Draw attention to any warranty imposed under the policy, major or unusual restrictions, exclusions
under the policy and explain how the contract may be cancelled.
4. Provide the client with prompt confirmation that insurance has been affected. If the final policy
wording is not included with this confirmation, the same shall be forwarded as soon as possible.
5. To not amend or alter any forms, documents or materials supplied by the Insurance Company or any
forms, documents or materials belonging to any customer, nor attempt or engage in any form of
forgery in respect of the documents related to any of the insurance products, applications, claims or
other documents submitted/produced before any customer or the Insurance Company.
6. To not induce or encourage customers to split policies to gain an unfair advantage for self.
7. Ensure that its client is aware of the expiry date of the insurance even if it chooses not to offer further
cover to the client.
8. Explain to its client their obligation as per the defined process of the specific Insurance Company to
notify claims promptly and to disclose all material facts and advice subsequent developments as soon
as possible.
9. Give prompt advice to the client of any requirements concerning the claim.
10. To put in best efforts, skills and services in conserving the insurance business already procured
through self.
The VLE-INS knowledge’s and declares that a breach or violation of this clause will entail action including
but not limited to monetary penalty, legal proceedings, termination and/or revocation of VLE-INS Certificate.

To follow at all times the Code of Conduct prescribed by the Regulator and any amendments thereto as
mentioned in the Guidelines issued by IRDAI/ VLE Insurance Handbook issued by CSC SPV.

2. REMUNERATION
In consideration of the VLE acting as a VLE-INS of the CSC SPV, CSC SPV shall pay the VLE-INS a
Commission at the rate and on terms specified under the Regulation on Common Public Service Centres-
2019.
Subject to applicable law, the CSC SPV reserves the right to reclaim/ claw back commission already paid to
the VLE-INS with respect to any policy which is cancelled or which lapses, for any reason whatsoever.

3. TERMS & TERMINATION

The term of this Agreement shall be for a period of 3 years. This Agreement may be renewed by CSC SPV
on mutually agreeable terms and conditions for a period mutually agreed by both the parties provided a
fresh agreement is executed to this effect. VLE-INS shall have no rights to demand any extension or
renewal of this Agreement.
Either party may terminate this engagement, without assigning any reason thereof, by giving 30 (thirty)
days’ written notice to the other party. Provided that this engagement shall at the option of the CSC SPV be
terminated, without requirement of any notice, in the event that the VLE-INS Certificate, authority or
registration with IRDAI or other relevant government, regulatory or supervisory authority expires and the
VLE-INS fails to renew the License within the prescribed time or the license is revoked or terminated.
RAP shall follow all guidelines/ instructions issued by CSC SPV/ IRDAI and non compliance of such shall
be construed as violations of terms & conditions.
On the suspension of the agency:
1. The VLE-INS shall use best endeavors to co-operate with the CSC SPV or others in any investigation
into any of the events referred above and shall provide all information, documents and assistance for
the purpose of such investigation.
2. The VLE-INS shall not solicit any business or provide any service under this Terms and Conditions
without the CSC SPVs prior written approval.

VLE-INS Signature:
Rebating: VLE-INS shall not demand or accept or share any remuneration in any form from / with any customer
to insure/ be insured on account of being a VLE-INS of the CSC SPV or for and on behalf of the CSc SPV in
connection with or incidental to it being a VLE-INS of CSC SPV. VLE-INS shall not pay or allow, or offer to pay
or allow, as an inducement to any person insure or to transact any business, any rebate of premium or any
inducement or consideration whatsoever other than as specified in the policy or as permitted by IRDA.

Publication: VLE-INS shall not use any Sales and marketing materials other than those as may from time to
time be supplied by the Insurance Company/ CSC SPV.

Intellectual Property: VLE-INS shall not use the trademarks, name or logo (or any part of the trademark, name
or logo) of the Insurance company / CSC SPV without prior written approval from the CSC SPV.
The VLE-INS shall comply with all applicable laws or record retention.

Limitations on VLE-INS authority: Without prejudice to any existing law, the authority of VLE-INS is limited to
the extent expressly granted herein. VLE-INS shall not bind CSC SPV in any way other than as expressly
authorized in writing by CSC SPV or incur any expenses or liability chargeable to CSC SPV except those
expenses expressly assumed by CSC SPV in writing.

Confidentiality: VLE-INS shall maintain confidentiality of information during the assigned period and even after
expiry of the license or obligation to perform function as a VLE-INS.

Declaration: I have fully read and understood the Terms and Conditions for VLE-INS (the "Terms"), stipulated
by CSC SPV appended to this application form and knowledge and agree that my appointment as a VLE-INS of
CSC SPV shall be subject to Compliance by me of the "Terms".
