Brief Incident Description

A las 07:00 horas, el trabajador Carlos Peñafiel (+) fue

instruido por el supervisor de turno realizar la perforación
en la labor Sub Level Stoping del Sub nivel -230, ya en el
sub nivel -230 recibe la confirmación de la tarea y
adicionalmente es instruido por el Ingeniero Seccional de DESPRENDIMIENTO
DE ROCA
desarrollos realizar voladura secundaria de sobre tamaño
de roca en Sub nivel -275.
Al tener dificultad para realizar la perforación de
taladros largos en Sub nivel -230 por la inclinación
de la estructura mineralizada, el trabajador Carlos
Peñafiel (+) junto a su ayudante trasladan el equipo
de perforación Raptor 44 al sub nivel -275 para
realizar la perforación de sobre tamaño de roca en
el sub nivel -275, concluye con la perforación y retira
el equipo para proceder con el carguío de explosivos
en los taladros perforados, en ese momento cae
una roca de aproximadamente 0,60 X 0,50 X 0,20 m
del realce, misma que impacta en la cabeza y tórax
del trabajador produciendo su deceso.

Consequences
• Traumatismo encéfalo craneal severo.
• Traumatismo torácico severo cerrado.
