Subject: Postponement of Modern Midlevel Service Provider Certificate Course

examinations

As per email letter dated 05/03/2019 received from Dr. Vijay Kandhewad, Joint
Director, P.D.E./NHM, Modern Midlevel Service Provider Certificate Course, examination
has been postponed. Accordingly Theory examination is rescheduled as follows:

Practical examinations will be held from 18th March, 2019 to 23rd March, 2019.

Accordingly last date of submission of exam form along with prescribed

Examination Fees has been extended. Students can submit their examination forms
upto 07th March, 2019, till 5.00 pm, at Head Office, MUHS, Nashik.
The last date of submission of examination forms cannot be extended further
under any circumstances. The candidate must submit Log book completion Certificate
along with the examination forms.
However, it is to be strictly noted that cut off date of eligibility for appearing for the
said examination remains unchanged i.e. 30th November, 2018.
