Question 1

(a) (1) employer need to give information, training and supervision to ensure, so far as
practicable, the safety and health at workplace of his worker. This can be apply by give
safety briefing daily and make in house or outsource training to his worker such as WAH
(Working at height) Rigger, first aider and others.
(2) To inform and prepare, so far as practicable, about operation, handling, storage and
transport of plant and substance. Employer need to make sure create SOP (Safe operation
procedure) of work and make sure all worker to follow the SOP that been given.
(3) To ensure, so far as practicable, prepare proper facilities for their welfare at work
such as toilet, proper office to work and all PPE (personal protective equipment) needed
to worker such as full body harness, safety boot and others.
(4) so far as practicable, to make sure the workplace is safe and no harm to worker such
as safe to going in and going out. The employer need to make sure the place have proper
safe access to their worker.

(b) (1) Safety officer is responsible for monitoring and assessing hazardous and unsafe
situations and developing measure to assure personnel safety
(2) Safety officer will correct unsafe act or condition through the regular line of authority,
although the safety officer may stop unsafe act when immediate action is required.
(3) Safety officer ensures the site safety and health plan is prepared and implemented.
(4) Safety officer advices the employer to make sure, so far as practicable, the workplace
is safe and no harm to health for all worker.
(5) Safety officer shall have received proper training within 3 years to renew the license
competency as safety officer
Question 2

(a) (1) Employer (i) to establish safety policy and safety and health committee at workplace
(ii) to make sure all worker get training to handle the machinery and create
SOP (Safe operation procedure), make sure all worker follow the SOP
Employees (i) to comply any instruction about safety and health from his employer
and safety officer
(ii) To cooperate with his employer and safety officer and give opinion
about safety and health about his self and other workers.

(b) (1) make sure have safety induction to new worker about hazard at workplace and daily
safety briefing.
(2) apply signage to hazardous area / workplace
(3) give proper training to worker.
(4) post the accident that happened from other company at bulletin board and explain the
hazard to worker
(5) give the worker proper PPE such as safety goggle, safety boot, glove and others.

(c) (1) add additional emergency stop button at machinery.


(2) Put extra guarding to all rotating machinery and put signage at hazardous part
(3) Apply SOP of handling machinery, brief to worker and make sure all worker follow
the SOP
(4) instruct system LOTO ( Lock out, Tag out) before maintenance process
(5) Apply PTW (Permit to work) before work process and locate the hazard of the work
(6) Supervise the worker when work in process
Question 3

(a) The guilty person is Company ABC (M) Bhd employer such as CEO, Head of
Department and factory manager.
(1) Employer not create ERP (Emergency respond plan) and no Safety and health manual
(2) Employer give job to underage to work at production
(3) Employer not check and get MSDS (material safety data sheets) of substance to know
the hazard of chemical and no SOP (Safe Operation procedure) when handling
material
(4) No inspection checklist for machinery such as forklift and no safety induction to new
worker
(5) System PTW (Permit to work) no assign and not been supervise by employer
(6) Employer not give proper training or send the worker to outsource training such as
ERT (Emergency respond training), First aider training, PTW training, and others

(b) (1) create a safety committee meeting between employer and employee at least interval 3
month and below
(2) make training program yearly to ensure proper training such as ERT, First aider and
others
(3) Apply PTW (permit to work) to any hazardous job to make sure the workplace is safe
to work and not harm for himself and others workers and been supervises by employer or
PIC (person in charge) such as factory supervisor, factory manager
(4) get and check substance MSDS to ensure the safety precaution for worker
(5) to monitor / supervise the inexperience worker and make sure all new worker been
give safety induction
(6) apply safety campaign to raise the safety awareness to all worker
(7) create safety master plan and SOP, brief to all worker and employer, make sure to
work safe follow the SOP
