“Level of Awareness of Pantawid Pamilyang Pilipino Program (4P’s) Beneficiaries in

Barangay Masili, Calamba City, Laguna”

Dear Respondents,

We are Senior High School students of Science, Technology, Engineering and

Mathematics Strand at Saint Benilde International School (Calamba) Inc. and currently
investigating on the “Level of Awareness of Pantawid Pamilyang Pilipino Program (4P’s)
Beneficiaries in Barangay Masili, Calamba City, Laguna”

We would like to request for your cooperation in the completion of the attached questionnaire.
There is no right or wrong answer and no known risk upon participation in the study. Please give
your honest opinion on each one of the statements. Please do not discuss your replies with
others. It is your opinion which matter. Even though it may be hard to decide, be sure not to
miss any question. The survey will take approximately 5-8 minutes.

The questionnaire is divided into two parts, the first part contains the socio-demographic
profile and the second part contains questions concerning the level of awareness on the
program.
