“It Made a Difference to Me”: A Comparative Case Study of Community Pharmacists’ Care
Planning Services in Primary Health Care

I. Introduction
1. The community pharmacy is a place where people can get complete health care
services.
2. Trust, communication, and relationship between patients and care providers are the
parameters that are undeniably useful for demonstrating the value of care planning.
3. The underlying principles of primary health care are equity, collaboration, and
community participation.
4. Based on the research result, pharmacists are expected to provide services that focus
more on the patients such as providing 24-hour health assistance.
5. Individual and private practice are needed to establish a proper care plan to achieve
the patient’s health goals.
6. The administration of primary health care is conducted by teams of health care
providers based on patients’ preferences.
7. Perspectives of the effectiveness and cost-effectiveness of pharmacy services are the
only values of care plans that can be observed .
8. In handling the acute conditions, community pharmacists are allowed to refer patients
to emergency departments and perform further examination with doctors.
9. The only stakeholders who depend on community pharmacists as providers of primary
health care services are the governments of Canada and Alberta.
III. Results
1. All sites involved in this study portray diverse contexts related to CACP services.
2. Site 4 had the most CACPs completed per month.
3. There were 5 patients from Site 2 who suffered from heart failure.
4. The results of the study indicated a sociomaterial exploration of value, which
emphasizes the importance of the social and collaborative aspects of care planning
services.
5. The patients explained that pharmacists’ services were more comfortable and faster
compared to physicians.
6. After experiencing CACP services, patients felt safe and calm.
7. Patients appreciated pharmacists who were informative and communicative when
performing care planning services.
8. CACP failed to change patients’ perceptions on the role of pharmacists which
focused only on filling prescription.
IV. Discussion
1. The areas of value related to primary health care services in this research were similar
to a research from Council et.al.
2. This research is the only research that focuses on importance of information sharing.
3. The results from current research and Talja’s research reveal various ways in
information sharing.
4. This current research is able to link the care planning services and increased
knowledge of community pharmacy services.
5. Although this study was conducted in Alberta environment, the results of this study
can reflect the similar issue of care planning services from other regions.
