G.9.3 / 08 dated 20th February 2008.
CMH Lahore Medical College was established in the year 2006. It is located near the premises of
Combined Military Hospital Lahore in the secure and serene environment of Lahore Cantonment. The
College occupies a total area of about 12 acres.
Inauguration of the New Campus was held on 16 January 2010 by the Prime Minister of Pakistan. The
new campus is a purpose built building, which is one of the most beautiful structures in the metropolitan
city. It is a fine blend of architecture, accessibility and comfort.
The campus consists of well equipped and state-of-the-art lecture rooms for each department, Anatomy
dissection hall, tutorial rooms, laboratories, library, computer lab, auditorium, cafeteria, book shop,
sports complex and a spacious parking area.
Library
The college library provides access to multi
disciplinary informatics in print and digital
formats. Library is being continuously improved to
provide valuable learning and research tools. It
regularly subscribes to a number of national and
international journals and magazines. Its premises
create ambiance of an intellectual sanctuary
wherein the students can concentrate on their
studies. The library also fulfils the teaching and
research needs of college faculty.

The price of Prospectus for both local and Foreign Students is Rs. 3,000/-.
The summary of Payment for Prospectus/ Admission Process fee and mode of payment is as under:
Note: Undertaking form is attached at the end of college prospectus or can be downloaded from the
college website: www.cmhlahore.edu.pk, or obtained from college admission office.
On the day of fee deposit – only for foreign student’s parents/guardians, who want to claim an exemption
of 5% advance income tax in fee structure
• Copy of passport of parent/guardian, as evidence to the educational institution that during
previous tax year, his/her stay in Pakistan was less than one hundred eighty three days.
• A certificate that he/she has no Pakistani source of income.
• The fee is remitted directly from abroad through normal banking channel to the bank account
of the College.
No amendment in the results/ data entered will be accepted after the last date of submission of
application. Results received after the closing date of submission of application will not be
entertained.
The applicant/guardian will be required to sign a “Declaration” confirming that the information
provided in the Application Form by him / her is correct.
The name of the student and father's name should be the same on all important documents and it
should be the one that has been endorsed on matric / O-level certificate. If different names (different
spellings, shorter versions etc) are endorsed, problems crop up when students register for different
exams / tests. If there is a discrepancy in the student's personal details (particularly their names) it
creates great difficulties for the students. Ensure that the name on student's passport is the same as
they have in their school record and the same name they will use for applying to different
universities.
Note: The college administration reserves the rights of admission and can cancel / reject any application at
any stage without assigning any reason.

Students admitted on local seats can make payment through banker's cheque only titled
"CMH Lahore Medical College".
Students admitted on foreign seats are required to remit the fee from aborad as per followng
details:
2. Local students can pay the fee through pay order in the name of CMH Lahore Medical
College and Institute of Dentistry.
3. Foreign students need to pay the fee mentioned in USD through banking
channel as remittance from abroad.
4. Foreign students need to pay the amount mentioned in PKR here in college through pay
order as per the account title mentioned above.
5. Above mentioned fee and charges are applicable to first year only.
6. The College administration does not take the responsibility of sending the
reminders to Parents/Guardians for the payment of fee.
9. If foreign students do not comply with the specified tax exemption conditions then
College will collect the advance tax as per prevailing rates.
14. If fee is revised by PM&DC at any time the College will charge the fee accordingly.

Hostel Policy
Refundable Hostel Security
All hostilities are required to deposit refundable hostel security amounting to Rs. 20,000/- (Twenty thousand
only) before joining. The security will be refunded after clearance.
• Hostel room rent charges amounting to Rs. 96,000/- (Rs. 8,000 p/m) will be collected in advance
annually along with college tuition fee.
• In case of joining during the academic year, full dues will be charged, which will be adjusted
on monthly basis.
• Hostel room rent of final year hostilities (passing out batch) will be charged on fortnightly basis at
the time of college final clearance.
Laundry Charges
Annual laundry charges Rs. 9,000/- (Rupees 750/- per month) will be charged. In case of long vacation
unutilized balance under this head will be refunded along with hostel security after hostel clearance.
• Filled hostel clearance form is required to be submitted to the Accounts and Finance Dept for the
processing of refund case.
• A fine of Rs 100/- per day will be charged if fee is paid within 31 days after due date.
• A fine of Rs 200/- per day will be charged if fee is paid after next 31 days after due date.
• If a student fails to pay after 61 days (without valid reason) of due date, he will be suspended
from attending classes till he/she pays the dues with fine.
• If a student does not pay after 91 days of due date without any written permission, he/she will
be suspended from the college rolls. In case of re-admission he/she will have to pay full
admission fee.
• Last date for depositing of annual dues is 31st August of every year for MBBS from 2nd year
onward.
General Rules
• The students must wear their identification cards on a lanyard at all times while in the college
premises and hospital.
• All students must maintain the decorum of the college through decent and respectable behavior.
Students are forbidden to sing, whistle, hoot, create noise, use abusive language, fight and indulge
in any unseemly behavior, politics or activity in the college premises at any time.
• No student is allowed to loiter and move aimlessly in and around college premises or waste his/
her time in college cafeteria extensively.
• Students have to be present in time, at any specified activity of the college.
• Student’s notices/ messages will only be displayed on the notice board placed by the college authorities
with the approval of the Principal.
Dress Code
The wearing of prescribed uniform has peculiar importance in medical profession and all students
must take pride in their identity. Any student found improperly dressed will be marked absent for the
whole day. A habitual defaulter in this regard will be dealt with appropriate disciplinary action.
Attendance
• The Attendance should be 100% in all lectures /
practicals / clinical sessions. As per requirements
of NUMS/UHS & PMDC, a minimum attendance
of 80% is MANDATORY “individually” in each
subject being taught in that year (in lectures /
practicals / clinical sessions), in order to qualify
for appearing in NUMS / UHS annual exam.
Peaceful Environment
The maintenance of a safe and secure environment in the campus, hospital and hostels has immense
importance in a professional institute. The college accords the highest priority to this vital aspect.
Examination
• Textbooks, notes and cellular phones are not allowed in the examination hall.
• Any student coming 10 minutes after the start of the paper will not be allowed to take the
examination.
• NUMS Professional Examination Admission form will be processed only after the
clearance from Accounts and Finance Department.
Hostel Discipline
Our excellent hostel buildings equipped with the modern facilities have been recently constructed to
ensure a student-friendly environment with comfortable living standards. The hostelites are expected
to enjoy their stay without any distraction and disturbance. Hostel wardens facilitate maintenance of a
congenial environment in a disciplined manner.

Disciplinary Actions
Various disciplinary actions taken by the committee according to the nature and degree of violation of
rules by the students after deliberations of the committee can be as under:-
• Withdrawal from College rolls.
• Debarment from all academic activities.
• Cancellation of hostel allotment.
• Cancellation of leave.
• Cash fines.
• Extra library sittings.
• Ban to visit cafeteria and hostel.
• Formal warning with information to parents / guardian.
• Disallowing to apply for university examination.
Lt Col Muhammad Saad Ullah Khan Sumbal is a graduate of Army Medical College Rawalpindi
(1985). Holds masters degree in administration from Quaid-e-Azam University Islamabad. An alumnus of
Aeromedical Institute Karachi, he served as a distinguished flight surgeon in Pakistan Air Force and Army
Aviation. He is recipient of postgraduate certification in Public Private Partnership Projects and Effective
Project Management from Pakistan planning and management institute Islamabad. During his illustrious
career in Armed Forces he has held various command and staff appointments of medical establishments
both inland and abroad. He has served as an evaluator and instructor of diverse healthcare establishments in
Punjab during his 3 year stint with Punjab Healthcare Commission. He has to his credit a tangible
contribution as a co-author in compiling reference manual of minimum service deliver standards (MSDS)
by Punjab Healthcare Commission.
Dr. Khan at CMH LMC & IOD is introducing the Masters and Phd programs in medical education,
bioethics, health economics and management. He is also providing support to faculty development at both
under graduate and post graduate clinical and basic sciences programs.
She is a graduate of Baqai Medical University and then worked in Pharmacology Department of Lahore
Medical & Dental College, Lahore. Dr. Rizwana did her MHPE from University of Lahore and published
three research papers. She also attended several workshops and conferences on Medical Education and
contributed to academic research & development at University of Dammam, KSA. She won the first prize
for her presentation in an international medical education conference.
She is a graduate of Rawalpindi Medical College and did her MHPE from Riphah International University.
Dr. Ayesha worked as Assistant Professor of Medical Education at Margalla Institute of Health Sciences,
Rawalpindi. She published three research papers and also presented paper in International Conference of
Medical Education OTAWA / ICME held in Abu Dhabi, 2018. Apart from that she also attended many
workshops, seminars, research conferences on medical education.
She graduated from Fatima Jinnah Medical College in 1985. She did her M. Phil in
2000 and joined teaching in Anatomy department at Fatima Jinnah Medical
College, Lahore and later taught in Postgraduate Medical Institute Lahore. She is

an author of 1 integrated journal of Anatomy “contextual journal of histology”
which integrate Histology and Embryology for undergraduate and post graduate
MBBS & BDS students. She also compiled & published museum atlases for
MBBS & BDS students. At present she is doing her Master in Medical Education & working on her thesis.
She has published many research papers and helped under & post graduate students in their research work.

He graduated from Nishtar Medical College in 1981 and served in Pakistan Armed
Forces for 33 Years and retired as Maj Gen in 2014. He had been teaching the
subject of Biochemistry in Army Medical College Rawalpindi since 1984.
Achieved M. Phil in 1988, Doctorate in 1999, FCPS Chemical Pathology in 2002 ,
FNAMS in 2003 and FRCP in 2007.
He is a member of the College of Physicians & Surgeons BMS Faculty since 2016 to date. He had the honour
of holding the post of Dean of Army Medical College, NUST for 7 years and Riphah International
University for 3 years and remained a member of PMDC from 2010 to 2017. Presently he is also Chairman
Technical Advisory committee of Pakistan Medical Research council from 2015 to date. He has numerous
National & International publications and so far supervised training of 42 M Phil, 13 FCPS and 13 Ph.D post
graduates in the subject of Biochemistry.
He has written three Books on Biochemistry and established a Research Centre for Biotechnology at Army
Medical College Rawalpindi. For his meritorious services he had received numerous National &
International Awards including 1. Best Teacher Award by Higher Education Commission of Pakistan,
2. President Gold Medal for the Best University Researcher Award of NUST 3. Best scientist of the year
Gold Medal by Pak Academy of Sciences in Medical Sciences and Hilal-e-Imtiaz Military. He is presently
working as Principal, Chief Executive Officer & Prof of Biochemistry of CMH Lahore Medical College and
Institute of Dentistry since Dec 2017.
Dr. Naheed Z. Razwi did her M.Sc. (Biochemistry) in 1975 from University of
Agriculture, Faisalabad, M.Phil. (Biochemistry) in 1983 from Postgraduate
Medical Institute, Lahore, and Ph.D (Biochemistry) in 2010 from Punjab
University, Lahore. Joined as Lecturer in biochemistry in Fatima Jinnah Medical
College, Lahore. Worked there for 11 years as Associate Prof. & Head,
Biochemistry department. She has worked in Dubai Medical & Dubai Pharmacy
College for Girls Dubai, UAE, for 171/2 years. She has been teaching Biochemistry at undergraduate and
postgraduate level since 2005 at various medical Institutes. Her scientific papers have been published in
various National and International scientific journals. Presently she is working as professor and head of
biochemistry department.

Brig (R) Abdus Sattar graduated from Quaid-i-Azam Medical College in 1979
and served in Pakistan Armed Forces for 31 Years. He has been teaching the
subject of Pathology (Chemical Pathology) in various Medical Institutes and
Hospitals. As an examiner in the subject of Pathology he is on the panel of
various Universities of Punjab.
He has been a member of Faculty of Chemical Pathology and an examiner of Fellowship Degree (FCPS)
awarded by the College of Physicians & Surgeons Pakistan. He was conferred "Life Time Achievement
Award" by Pakistan Society of Pathology for his meritorious services in the field of Pathology. He has
more than 30 publications on his credit in various reputable journals.

The department museum is well equipped with required models / specimens and equipment for practical
demonstrations and tutorials. Field visits to places of public health importance, household surveys and
research work is being arranged regularly.
Col (R) Dr. Muhammad Ashraf Chaudhry is Professor and Head of Community
Medicine Department at CMH Lahore Medical College. He is highly qualified,
and most experienced Public Health Professional. He has served in Armed Forces
for about 25 years on various teaching and administrative appointments. He is
excellent and reputable teacher and has vast experience of teaching the subject of
Community Medicine / Public Health both to undergraduate and postgraduate medical students. He has
written several articles in various newspapers on important public health issues of Pakistan in order to raise
awareness among the public. He has created a number of pamphlets / brochures / handbills and literature on
important public health issues, to create awareness among the community regarding various health
problems. His more than l6 articles have been published in various reputable journals.

Professor Rana graduated from Nishtar Medical College in 1978, and served
Pakistan Army for 03 years. Started his career from Quaid-e-Azam Medical
College as Demonstrator in Forensic Medicine & Toxicology. He acquired

in 1989.
He served at King Edward Medical College, for about 18 years as Assistant, Associate and Professor. He
has vast teaching experience in the specialty and has about 15 publications on his credit. He has been
chairman and convener of DMJ (diploma) and M. Phil degree programmes. He has been on the panel of
various universities of Punjab as examiner in the subject of Forensic Medicine since 1986.

This centre is a referral institute and receives cases from all CMHs, Naval, PAF and Civil hospitals. This
centre is renowned for its research activities. Apart from invention of artificial ulna, the electric hand
(shulja), invention of HULS centre was awarded 1st prize ($6000) in international hand surgery conference
at Hong Kong in 2012. Apart from clinical training HULS Centre inculcates aptitude of research in the
medical students visiting HULS during clinical rotations.
Prof Dr. Arif Qayyum Khan is a graduate of King Edward Medical College
Lahore. He received his post graduate training in Internal Medicine, Gastroenterology and Hepatology in
USA. He is a Diplomate American Board of Internal Medicine and Gastroenterology. He is a Fellow of the
American College of Physicians, Fellow of the Royal Colleges of Physicians (London, Glasgow and
Edinburgh), Fellow of the American College of Gastroenterology, Fellow of American Gastroenterological
Association, and Fellow of the American Society for Gastrointestinal Endoscopy and Member American

Capabilities
Faculty members are assisted by competent and skilled paramedical nursing staff. The department is
recognized by College of Physicians and Surgeons Pakistan for Post-graduate training in the field of
Anaesthesiology. The department is also a recognized and approved Centre by Pak Army for training of
nursing and paramedical staff. The department of Anaesthesia, Intensive Care and Pain Management
consists of a main operation theatre complex, Gynaecology and Obstetric OT, and a main ICU.
Main OT Complex
It comprises of seven operation rooms to provide facilities to perform General Surgery, Orthopaedic
Surgery, Paediatric Surgery, Neurosurgery, Vascular Surgery, Plastic and Reconstructive Surgery, Thoracic
Surgery, ENT Surgery, Gynae & Obst Surgery.
Each operation room is equipped with modern anaesthesia work station and monitoring device. A Central
Sterile Supply Department is an integral part of the main operation theater complex.

regulation and instructions issued by the administration from time to time. Moreover I declare that I have been

I have to ensure at least 80% attendance “Individually” in each subject being taught in that year (in
lectures / practicals / clinical sessions) and 50% marks in the class tests / send up examination;
otherwise the college administration may not allow me to appear in the university examination. The
attendance of subjects not being examined in the same year will “also be carried forward till the year
of examination of that subject and will be included in minimum 80%” attendance calculation for
being eligible to appear in university annual examination.
If I get detained in the previous class, I will pay 100% annual fee (Tuition and other charges) for the
existing class irrespective of the No of failed subjects. .
Any type of leave, including medical leave, granted to me during the course of studies will not condone
a deficiency in attendance.
The college administration can take any disciplinary action, including expulsion from the college, on
violation of discipline and in such cases the decision of the principal will be final.
I have adequate financial resources to support my studies at the college. I fully understand that fee,
once paid will be refundable as per PM&DC regulations, where applicable, I will not object to any additional
charges levied in the future by the Government, University or College.
I, solemnly declare that I have not been expelled from any medical college for any reason what so ever
and I have not been disqualified on completion of admissible chances to appear in 1 st professional MBBS / BDS
from any Medical / Dental College of Pakistan.
