1. I am the handling counsel of MARIA LUISA RAMOS, the

petitioner in Civil Case No. ____________________ (For: Declaration of
Nullity of Marriage), entitled “MARIA LUISA RAMOS v. MACARIO DE
MESA ”;

2. I have personally verified petitioner’s residency, specifically at

12345 TAFT AVENUE, MALATE, MANILA, and confirmed that the
petitioner had been residing thereat, for more than six (6) months prior
to 2 OCTOBER 2019 , when the instant petition was filed; and

statements and for whatever legal purpose this may serve.
