without going through the Bank’s card transaction authorization and security parameters:

Once the Bank tags or classifies my identified Card/s under UNRESTRICTED status,
1. Any and all transactions made with or through the Card will be allowed;
2. The Bank will have no means to and cannot monitor any and all transactions made through or
with the Card/s, including but not limited to those made at any and every kind of merchant or
establishment, located in the Philippines or abroad, or those made via internet, website or on-
line, or those made through mail order or telephone order (MOTO);
3. The Bank will not and cannot send SMS or text messages to me, call me, send any emails, or
send any form of communication or notification to me in relation to any and all Card
transactions;
4. The authorization processing of all transactions made through or with my Card/s shall be based
on my full available credit limit as well as any subsequent increases in my credit limit, regardless
of whether the increases were initiated by me or by the Bank;
5. The security features of the Card/s will be cut off;
6. As a principal condition for the tagging of my Card/s under UNRESTRICTED status, I shall be
absolutely and unconditionally liable and pay for any and all transactions made through or with
my Card/s, including any and all interests, penalties and charges that may be assessed in
connection with such usage or transactions. I hereby hold the Bank free and harmless from any
and all liabilities arising out of the use of the CARD/s.

I am signing and submitting this letter-request after having carefully read and understood the foregoing
terms and conditions for tagging my Card/s under UNRESTRICTED status and after the same have been
clearly explained to me by a Bank personnel, who has particularly called my attention to the potential
risks and my unconditional liability to pay of having my Card/s tagged UNRESTRICTED. Thank you.
