structure specified below. If the format of this Annex does not accurately reflect the cost model
associated with your proposed solution, complete it to the best of your ability and propose your
alternate cost model separately.

PART A: Build Solution Requirements Pricing

Solution and Implementation

Detail the solution pricing for each component of the solution in the table below. If the proposed
solution contains alternatives for the GOB to consider (ie: Software-as-a-Service, Service Provider
Hosted Solution, Custom Developed, COTS, etc.), clearly diferentiate each alternative by completing
these tables for EACH alternative proposed.

Include, at a minimum, all requirements discussed in Section B: Requirements and any project
management services that are proposed.

NOTE: For evaluation purposes, the GOB would like to keep the cost models for
Solution/Implementation Pricing and Hosting Pricing separate. Hosting pricing should be detailed in
PART B: Hosting Services Pricing below.
