I, MARITES P. LEGASPI of legal age, Filipino, and a resident of 100 J.

Santiago St. Malanday Valenzuela City, after having been duly sworn to in
accordance with law do hereby depose and state that:

1. I am a holder of a valid Philippine Passport sometime 2006;

2. That on 2008, due to flood and typhoon occurred, the said passport
was loss;

3. Diligent efforts were exerted by the herein affiant to recover the same
but to no avail the said passport is now beyond recovery and thus
considered lost for all purposes;

4. I am executing this affidavit to attest to the truth of all the foregoing and
for securing a new passport.
