Subject: Coordination for Surveying Grade 6 to 10 Students and Their Mothers

Greetings,
We are the Fairplay For All Foundation, a registered non-profit, non-stock, and non-
government organization based in Quezon City that believes in levelling the playing field,
helping to create a more just world where families in poor communities are not destined to be
trapped in the poverty cycle, but are empowered to break it themselves.
Our foundation is doing an exploratory research about Social Intervention in an
Urban Poor Community: Developing Social and Emotional Support for the Promotion of
Mental Health in Payatas. We intend to explore different kinds of effective social
interventions that can help mitigate the effects of adverse childhood experiences to students
and their mothers. We are going to conduct this survey to Justice Cecilia Munoz Palma High
School to be a point of comparison to our chosen students that will undergo these social
interventions that we will employ. We will be surveying the selected students twice, once this
year, this 3rd week of September, and in 2020 by the 2nd or 3rd quarter.
In this light we would like to ask for an endorsement from your honorable office to
Justice Cecilia Munoz Palma High School for us to conduct a survey of the students and their
mothers. We would like to conduct surveys to 75 students within Grades 6 to 10 and also the
mothers of these students. We would like to request that Justice Cecilia Munoz Palma High
School would provide the list of all Grade 6 to 10 sections in order to determine the students
that would be included within the surveys, as we will employ a random stratified sampling
through a Kish Grid to give equal chances to all the Grade 6 to 10 students. The survey will
last for 30 to 40 minutes. The students chosen would also inform their mothers that they were
chosen for the survey that will be conducted. The survey will measure seven (7) Key
Performance Initiatives, which are; ACEs, risk of depression, well-being, learned optimism,
growth mindset, self-esteem, and resilience1.
We intend to improve the conditions of the students that is under our Foundation, we
are open to share the reports of these interventions after we are done with the project and this
might be of reference to policies or projects that different schools that would want to employ.
