Location: Water treatment plant

Safety issues observed:

1. Internal temporary road condition was not acceptable. They become
muddy and slippery due to rain and drainage of rainwater from pits.

2. Water had been collected inside the DI bends stored in the site. Tool
had been kept unsafely in the bend that may damage the protective
coating of the bend.

3. Shoring arrangement at CCT

area is unsafe.

4. Housekeeping at site office /

labor sanitary facilities were not
up to acceptable level.
5. The wire mesh fence along marshy area had been loaded with
heaps of soil and rock.

6. Shoring had not been provided when working in the deep

excavated pits.

7. Helper had not been provided to the tipper truck. Maneuvering

construction vehicles in the site without helper is very unsafe.

8. Stray dogs are living in the site premises.

9. Some of the bar benders and bar binders worked without wearing safety gloves.

10. Hand rails had not been provided to the working platform during wall concreting.
11. Garbage bins had not been labelled.

Location: Bill no 02 – Owilana road

Safety issues observed:
12. Unavailability of an emergency vehicle throughout the road closed time.

13. Pipe laid sections had not been temporarily reinstated. That road surfaces were slippery. It was
unsafe to the public.

14. Above ground telecom cables had been diverted unsafely using barricade tapes.

Location: Bill no 02 – Pattivila Makola road, rock chemical blasting site

Safety issues observed:
15. Daily site cleaning had not been carried out.

16. The safety jackets were worn out. New safety jackets had not
been provided to workers.

17. Provided safety sign posts were not sufficient.

18. Poor light on blinking lights. Switches in blinking lights were not working.

19. Water stagnation in barrels and water tanks that allows mosquitos to breed.

20. Temporary reinstatement along Pattivila-Makola road was outdated. Lots of potholes had been
formed. Rain water had been collected in that potholes.

Location: Bill 01 – Access road construction area

22. Earthing arrester shall be installed in this plain area. (paddy field).

The pipe trench was not securely covered or fenced off when we
arrived the location. The pipe laying has been stopped due to a
rock encountered in the trench.
