1.Pursuant to DepEd Memorandum No. OM-OAGA-20 19-00176 otherwise known as


"Administration of Survey Form for Selected Schools Heads for the Possible Inclusion of
Alternative Learning System in the School Based Management System (SBM)" the
selected schools with ALS program are hereby required to fill up the survey form attached in
this memorandum and submit to the Division ALS Focal Person on or before 12:00 noon of
July 31, 2019.

2. This survey is in line with the plan for the inclusion of Alternative Learning System (ALS) in
the School-Based Management, the Alternative Learning System (ALS) Task Force together
with the School Effectiveness Division (SED) under the Bureau of Human Resource and
Organizational Development (BHROD) will conduct survey to school heads who are
implementing ALS in their schools. The objective for the conduct of survey is to come up with
a comprehensive database that will allow an accurate analysis of the characteristics of schools
implementing ALS with the intention to establish a model school to implement the proposed
School-Based Management

In line with the plan for the inclusion of Alternative Learning System (ALS) in
the School-Based Management, the Alternative Learning System (ALS) Task
Force together with the School Effectiveness Division (SEE)) under the Bureau
of Human Resource and Organizational Development (BHPOD) will conduct
survey to selected school heads who are implementing ALS in their schools.
The objective for the conduct of survey is to come up with a comprehensive
database that will allow an accurate analysis of the characteristics of schools
implementing ALS with the intention to establish a model school to
implement the proposed enhanced School-Based Management.

In this connection. the Regional ALS Focal Persons are requested to print and
distribute the attached survey form to the Division ALS Focal Persons. The
Division ALS Focal Persons will administer the survey to selected school
heads based on the attached list, and collect the same and send back to the
Regional Focal Persons.

Kindly advise the Regional ALS Focal Persons to send the compiled
accomplished forms to ALS Task Force on or before July 31, 2019 through
courier. Expenses relative to the conduct of this activity shall be charged to
2019 ALS Support to Operations Fund.

Good day! As part of the on-going discussion and planning for the inclusion of the
Alternative Learning System (ALS) in the School-Based Management (SBM), this survey
form and all its questions were developed which aims to inform the ALS Task Force and the
School Effectiveness Division (SEE)) under the Bureau of Human Resource and
Organizational Development (BHROD) about the implementation of AIS in some public
schools to be used as basis in crafting the enhanced 'SBM.

The specific intention of this exercise is to come up with a comprehensive database that will
allow an accurate analysis of the characteristics of schools implementing ALS, with the
intention to establish a model school to implement the proposed enhanced SBM. in this
regard, your accurate and honest responses will greatly help in making the ALS program
better.

Please be assured that all the information you will provide will be treated with utmost
confidentiality. It will be included in a national ALS database without personal identifiers.
Your personal details are only being asked for verification and/or clarification purposes, but
it will not be made available to the general public. Your responses will not be used in any
performance evaluation or investigation, nor will it negatively affect your individual status
within the organization in any way.
