NOTARIAL HEARING

FIRST CALL –

K: Good morning, your honor. _____, respectfully appearing as the


Petitioner.

I am ready to establish jurisdictional facts and other qualifications for


my petition for notarial commission.

May I proceed, your honor?

MARKING OF EXHIBITS –

K: I would like to request the marking of these following exhibits, your


honor:

As EXHIBIT A the PETITION FOR NOTARIAL COMMISSION


DATED _____, which is already attached to the records of the case,
your honor.

As EXHIBIT A-1 the NAME and SIGNATURE of the petitioner.

As EXHIBIT B the ORDER OF THIS HONORABLE COURT


DATED SEPTEMBER 12, 2019 finding the instant petition sufficient
in form and substance and ordering the publication of such order and
the setting of the date of hearing on _____ AT 830 IN THE
MORNING.

As EXHIBIT C the CERTIFICATE OF POSTING dated


_________ executed by ______________________ which I reserve to
be marked later on, your honor, as I still have to obtain a copy of this
from the process server.

As EXHIBIT D the AFFIDAVIT OF PUBLICATION executed by


_____. I would like to submit the original document to be attached to
the records of this case, your honor.

As EXHIBIT D-1 the NEWSPAPER CLIPPING DATED _____. I


would also like to attach the original of said newspaper clipping to the
records of this case, your honor.

As EXHIBIT E the PROCESS SERVER’S CERTIFICATE


certifying that the process server personally went to the office of the
petitioner at _____. I also reserve the marking of this exhibit, your
honor, as I still have to obtain a copy of this from the process server.

As EXHIBIT F the CERTIFICATE OF MEMBERSHIP IN THE


PHILIPPINE BAR issued by the Office of the Bar Confidant of the
Supreme Court dated _____, stating that I, the petitioner is a full-
fledged member of the Integrated Bar of the Philippines. The original
copy is already attached to the records of the case and made an integral
part thereof, your honor.

As EXHIBIT G the CERTIFICATION issued by the OFFICE OF


THE BAR CONFIDANT dated _____, stating that I, the petitioner
has no pending administrative case before the said office.

As EXHIBIT H the CERTIFICATION from the Integrated Bar of


the Philippines _____ Chapter, signed by two (2) executive officers
dated _____, as proof of my good moral character.

As EXHIBIT I the CERTIFICATION or clearance issued by the


OFFICE OF THE CLERK OF COURT OF THE REGIONAL
TRIAL COURT OF _____ dated _____, stating that I the petitioner
have no pending criminal or civil case before the RTC of _____.

As EXHIBIT J the CERTIFICATION or clearance issued by the


OFFICE OF THE CLERK OF COURT OF THE MUNICIPAL
TRIAL COURT OF _____ dated _____, stating that I the petitioner
have no pending criminal or civil case before the MTC of _____.

As EXHIBIT K a CLEARANCE from the NATIONAL BUREAU OF


INVESTIGATION dated _____ stating that I the petitioner have no
derogatory records.

As EXHIBIT L a PROFESSIONAL TAX RECEIPT with PTR No.


_____, proving that I have paid the required Professional Tax for the
calendar year.

As EXHIBIT M an IBP official receipt with OR No. _____ dated


_____ proving that I have paid the required membership dues to the
Integrated Bar of the Philippines.

As EXHIBIT N, the three (3) un-retouched passport size photos of


me, taken within 30 days prior to the filing of the instant petition, with
my signature at the bottom of each photograph

As EXHIBIT O, my 3 specimen signatures; and

As EXHIBIT P, the Official Receipt showing Payment of Filing Fees.
