AFFIDAVIT OF CANCELLATION OF BUSINESS

I, SUSAN G. TRUMATA , of legal age, single and a resident of Annafunan

East, Tuguegarao City, Cagayan after being duly sworn in accordance with law,
depose and state:

1. That I am the registered owner of a business under the trade name

Jullien’s Panciteria;
2. That the above-said business was duly registered at the
Department of Trade and Industry (DTI) on 6 July 23, 2018 and was
issued Certificate of Registration with No.05474822;
3. That at present I am causing the cancellation of the said business due
to economic reasons;
4. That as far as my personal knowledge as the registered owner, I attest
to the fact that Jullien’s Panciteria , has no standing financial
obligation to any individual, public or private corporation or entities;
5. That I am executing this affidavit to attest the truthfulness of the
foregoing declarations under penalties of perjury and to support
respectfully my request to the Office of DTI to CANCEL Certificate of
Registration, No. 05474822 moreover to serve all legal intents.
