AFFIDAVIT OF BEING SINGLE

I, SATURNINO A. BULAQUI, JR, of legal age, single, Filipino, and a

resident of Annafunan East, Tuguegarao City, Cagayan, after having
been duly sworn to in accordance with law, hereby depose and
say:

1. That I am a candidate for Soldier Course in the Armed Forces

of the Philippines;

2. That I was born on April 12, 1997 and one of the children of
spouses Saturnino D. Bulaqui, Sr. and Julieta A. Bulaqui;

3. That I am still single, as I have never contracted any

marriage with another person up to the present;

4. That I am executing this affidavit to attest to the truth of the

above statement for whatever legal purposes that this
affidavit may serve.
