4. A copy of the Internal Revenue Service determination letter, if received. If the determination
letter has not yet been received, please provide us with the entity's FEIN. If unavailable, a copy can

5. A copy of the Application for Recognition of Exemption (IRS Form 1023, 1023-EZ, or 1024), if
submitted. If unavailable, a copy can be requested from the IRS at (877) 829-5500 or
https://www.irs.gov/uac/about-form-4506a.

Please respond within 30 days from the date of this letter and include a copy of this letter
with the above-described documents. Your failure to respond will lead to this office making a
referral to the California Franchise Tax Board and may also result in your organization losing its
tax-exempt status.
