30. Mrs. Johnson is a 72yearold woman hospitalized for left lower lobe pneumonia.
She complains of a persistent cough and occasional pain in her left chest associated
with coughing, fever, and shortness of breath. Prior to this admission, she has been
healthy and independent. Upon assessment, you find her mildly dyspneic,
occasionally pausing in the middle of a sentence to breathe. Her respiratory rate is
28 breaths per minute. Her pulse is 100 per minute and regular; blood pressure is
140/90 mm Hg. Her skin is warm, dry, and pale pink with brisk capillary refill. You
do not note any edema. She is receiving oxygen by nasal cannula at 6 L/min.
31. Joan McIntyre, age 72, is in the medical intensive care unit, diagnosed with severe
mechanical ventilation. Efforts are being made to wean her from the ventilator, but
she has been unable to breathe on her own for any length of time. She has written
notes asking the staff to “let me go” the next time she fails to be weaned. Her only
daughter is by her side and asks that all measures to save her mother’s life be
initiated.
