JOINT-AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, xxxxxxxxxxxxxxxxxxx widower, both with address at San Basilio, Sta Rita,
Pampanga, respectively, after having been duly sworn to in accordance with law, hereby
depose and say, that:

1. We are the neighbors of xxxxxxxxxxxxxxx.

2. We have known her for more than 20 years.

3. We also know that she has been using xxxxxxxxxxxxxx as her nickname since we
have known her.

4. We are attesting to the fact that the person by the name of

xxxxxxxxxxxxxxxxxxxxx as appearing in the MARRIAGE CONTRACT with Register no
9300112 (Annex A), the Certificate of Birth of xxxxxxxxxxxxxxxxxx with Registry No.
9700294 issued by the Philippines Statistics Office (Annex B), the POSTAL ID with
PRN C17180016945 (Annex C) and POLICE CLEARANCE (Annex D) issued by the Sta.
Rita, Municipal Police Station and xxxxxxxxxxxxxxxx as appearing in the Certificate
of Live Birth of xxxxxxxxxxxxxxxx (Annex E) with Registry No. 2001-1308 REFERS TO
ONE AND THE SAME PERSON.

5. We are not in any way related to any of the foregoing person within the fourth civil
degree either by consanguinity or affinity, nor do we have any pecuniary interest in
this matter whatsoever.

6. To remove confusion as to the two names, we hereby declare that the name
xxxxxxxxxxxxxxxxxx and xxxxxxxxxxxxx refer to one and the same.

7. We are executing this affidavit to attest to the truthfulness of the foregoing

statements and to establish the real identity of xxxxxxxxxxxxxxx and for all legal
intents and purposes it may serve.

8. As such, we execute this Joint Affidavit to attest to the truth of the foregoing facts
and for any other legal purpose that this Affidavit may serve.
