Dr Malik is a faculty member at Indiana University School of Medicine,

INFERTILITY EVALUATION & TREATMENT


For some couples, becoming pregnant is a challenge. Infertility is the inability
to get pregnant despite having frequent, unprotected sex for at least a year.
Although many couples are really subfertile rather than infertile, they are
often considered infertile when they haven't conceived despite trying for one
year. This is usually the point to start seeking medical support as the monthly
chance of pregnancy is 4 percent or less after one year. Faster evaluation
may be appropriate for women over the age of 35, when there are known
medical conditions that interfere with fertility, or if trying to conceive causes
signicant stress.
Infertility is not just a woman's problem. Causes can involve one or both
partners. In fact, the incidence of sperm problems is on the rise, accounting
for up to half of infertility causes, according to some studies. In other
instances, couples want to plan ahead to ensure fertility after a disease, such
as cancer.

TREATMENT METHODS AND TECHNIQUES
Male Evaluation
In up to 30 of the infertile couples male factor is important. We evaluate this
by performing a SEMEN ANALYSIS.

IVF
In Vitro Fertilization: We would stimulate ovaries to produce many eggs,
which are then retrieved and inseminated with partner's sperms. Fertilized
eggs now called an embryo is cultured in highly controlled incubators.
Resulting embryos are replaced into the uterus for implantation.

ICSI
Intracytoplasmic sperm injection is an advanced technique utilized to achieve
fertilization by injecting a selected sperm into a mature egg. This specialized
technique can be especially useful for couples with very low sperm counts.

Assisted Hatching
The embryo after culture is encased in a protective layer called zona pellucid.
In selected cases we open this protective coating to give the embryo
increased chances of implantation.

Blastocyst Culture
In selected cases, we culture the embryos to an advanced stage and perform
a day 5 transfer thus allowing us a greater ability to select the best embryos
and improving the success rates of implantation.

Embryo Cryopreservation
Embryo Cryopreservation also called embryo freezing allows us to freeze any
extra embryos in a cycle and use them for transfer in a subsequent 'frozen
cycle'. With improved freezing techniques some studies are reporting better
pregnancy rates in selected patients in frozen cycles as a result of more
receptive endometrial lining.

Semen Cryopreservation
Sperm freezing frequently utilized in cancer patients, for sample to be utilized
once battle against cancer has been won. Many patients undertake social
freezing such as busy work schedule, frequent travelling, overseas
deployment, where sperms sample can be thawed and used for insemination
at a time when female partner is ready. In addition sperms retrieved after
vasectomy, and testicular sperms obtained by TESE can be frozen and used
later on as well. Research shows that sperm that have been frozen and
thawed have promising fertilization rates and are no more likely to result in
birth defects than freshly ejaculated sperm.

Fertility Preservation
Oocyte (egg) freezing is an integral part of assisting cancer patients preserve
their reproductive potential before they start chemotherapy that may
damage the remaining eggs. In addition vast number of career oriented
women are preserving their eggs at a younger age when eggs have a lesser
chance of genetic mutations, and before their ovarian reserve gets
diminished as a result of age. We use an advanced technique called
vitrication to freeze the eggs, that gives us higher survival rates upon
thawing the eggs, and subsequently better pregnancy rates than many other
centers, that may be using older techniques.

Pre-Implantation Genetic Counseling and Screening


With a family history of genetic disease, advancing age, previous IVF failures,
PGD/PGS can help in having a healthy successful pregnancy.
Recurrent Pregnancy loss
Loss of pregnancy in early weeks frequently has an underlying anatomical or
endocrinological reason. Identifying and xing these defects, can improve
your chances of a successful pregnancy by two to three folds. Our physicians
will undertake a thorough evaluation and make a plan for an improved
outcome for your future pregnancies.

Pregnancy after having a


tubal ligation
