INFORMATION

The Office of the City Prosecutor thru the undersigned Assistant City Prosecutor
accuses WILSON AGOR MALINAO of the crime of VIOLATION OF SECTION 5,
ARTICLE II of REPUBLIC ACT NO. 9165, committed as follows:
“That on or about 6:30 o’clock in the evening of September 6, 2019 at
Barangay Dubinan West, City of Santiago, Philippines and within the jurisdiction
of this Honorable Court, the above-named accused, in consideration of One (1) piece
of Five Hundred Pesos (Php500.00) marked bill with Serial Number DV437222,
did then and there willfully, unlawfully and feloniously sell and deliver One (1)
piece heat sealed transparent plastic sachet with markings “MBM 09-06-19” to a
PDEA Agent 0.039 gram of Methamphetamine Hydrochloride locally known as
“Shabu” a dangerous drug without authority or license to do the same.”

“That on or about 9:45 o’clock in the evening of September 6, 2019 at Barangay

Mabini, City of Santiago, Philippines and within the jurisdiction of this Honorable Court,
the above-named accused, in consideration of One (1) piece of One (1) Thousand
(Php1,000.00) pesos marked bill with Serial Number CZ876741, did then and there
willfully, unlawfully and feloniously sell and deliver One (1) piece heat sealed transparent
plastic sachet with markings “LCP-9-6-19” to a PNP Poseur-buyer 0.053 gram of
Methamphetamine Hydrochloride locally known as “Shabu” a dangerous drug without
authority or license to do the same.”

I N FO R M A T I O N
The Office of the City Prosecutor thru the undersigned Associate City Prosecutor of
Santiago City hereby accuses MARIO ORTIGUIERO of the crime of RECKLESS
IMPRUDENCE RESULTING TO SERIOUS PHYSICAL INJURIES AND DAMAGE TO
PROPERTY defined and penalized under Article 365 in relation to Article 263 of the Revised
Penal Code, committed as follows:

“That on or about the 21st day of April, 2019 at around 8:15 o’clock in the evening,
along Malini Road, Purok 5, Calaocan, Santiago City, Philippines and within the jurisdiction
of this Honorable Court, the above-named accused, being then the driver of a V2-red Motorstar
Star X125 tricycle (kulong kulong) bearing Plate Number 1192JZ, did then and there willfully,
unlawfully and feloniously manage to drive in a negligent, careless and imprudent manner,
without due regard to traffic laws, rules and regulations and without taking the necessary
precaution to prevent injuries to persons, while travelling at said road heading towards four
lanes, encroached to the opposite lane and sideswiped the left portion of the approaching V-1
Black Suzuki Drive single motorcycle bearing Plate Number 9965BU, driven by Gian Carlo
Taguinod y Ladrigo, and as a result thereof, the said Gian Carlo Taguinod y Ladrigo sustained
serious injuries which requires 45 days healing period and damage to his motorcycle in the
total amount of P9,050.00, to his damage and prejudice.
