3 days before the patient’s admission, he noticed that he easily gets tired after doing small works

on his house. The patient’s wife stated that she just increased the fluid intake of the patient to 6 glasses
of warm water or more per day and drank an herbal medicine called serpentina and ate 3 insulin plant
leaves on that day.
4 hours prior to admission, Sunday Morning the patient stated that he does not feel any
weakness nor tiredness after waking up, however upon walking to the bathroom to micturate, he feels
dizzy every 2 hours with constant weakness. The patient said that he just sat on a chair and drank a
glass of warm water.

IV. PAST MEDICAL HISTORY

At the age of 5, the patient had a hyperthermia with a temperature of 38.2 °C. As a treatment,
the mother of the patient let him go to a faith healer in their province. The duration of his sickness is 1
week.
At the age of 12, the patient acquired mumps with mild fever; his father put a blue dye with
natural vinegar and house of wasps on the affected area and taken paracetamol for the mild fever. The
span of the patient’s mumps is 4 days.
At the age of 16, the patient suffered from a broken ankle with a severity of 5 with 10 as the
worst because of playing basketball, as a treatment his mother let him go to a faith healer and the
healer just massaged the area of the broken ankle using oriental oils and herbs. The duration of the
injury is 4 weeks.
At the age of 39, The patient was diagnosed of diabetes mellitus with sugar levels of 60-130
mg/dL at San Jose Hospital & Trauma Center in G.M.A., Cavite. The attending physician also ask the
patient to have a kidney and heart check-up to make sure those systems are not yet affected of the
disease.
The patient at the age of 17 suffered from a 3 inches’ laceration parallel to the cleavage lines of
the right arm because of a fall from a fence on their neighborhood. His mother brought him to the
nearest lying in clinic in their barangay as treatment into his injury. The patient had simple running
stitches on his right arm and the wound completely healed in 1 month.
The patient has no allergies on any kind of food, drugs, animals and plants.
 The patient stated that he experienced having hyperthermia once every 5 years in his
adolescence with a temperature ranging from 38.0 °C to 38.7 °C. To lower his body temperature, he
has taken 500 mg of acetaminophen 3 times per day as their barangay’s pharmacist’s prescription. His
hyperthermia lasts just for 3 days.
The patient had a cough and cold once a year, as a treatment he drinks the juice of freshly
picked oregano from his backyard 2 times a day after meal. The duration of the cough and cold of the
patient is 4 days annually.
The patient is taking 500 mg of metformin 2 times per day every after meal in the morning and
evening as a lifetime maintenance medicine to control high blood sugar level.
The patient said that he is not full in immunization; he only had measles vaccine when he was
1-year-old in a health care center in a province.
The patient’s last assessment was on his urine output related to his disease diabetes mellitus;
according to the patient’s wife, the urine color was ranging from transparent yellow to dark yellow. Also
the patient’s blood pressure and creatinine blood test was taken at Theramedica Health System Inc. in
Governors Drive, Old Bulihan, Dasmarinas City.
