POI – You are only required to pay the Hockey Canada Insurance once per season. If you play
with the Cranbrook Shooters you may submit a completed Proof of Insurance form with your
KEWHL registration and deduct $40.00 from your fee.

The Kootenay East Women’s Hockey League is a house league program. Please note that great effort is made to
balance the player skill level/experience on each team. Therefore the KEWHL cannot guarantee you will play on
the same team as a friend or family member. League games are played at Western Financial Place on Tuesdays
from 8:45 to 10:00 pm & at Memorial Arena on Wednesdays from 8:45 to 10:00 pm and on the occasional Sunday
from 6:00 to 7:30 pm.

I the undersigned, being of the age of majority, hereby agree to participate in the Kootenay East Women’s Hockey League program for the
2019-2020 season and I assume all risks and hazards to such participation in this program including, but not being limited to, the playing of
hockey, whether in practice or game format and transportation to and from said practice or game, and I hereby forever remise, waive and
release the KEWHL, its organizers, sponsors, coaches, assistants, trainers, and supervisors from any and all claims for damages and or,
injuries arising out of my participation. I authorize emergency medical or surgical treatment for myself if such treatment is required.
I acknowledge that:
1) All registration must be paid in full prior to September 30, 2019. Please make cheque payable to “Cranbrook Women’s Rec Hockey League”
2) No player will be permitted on the ice unless paid in full
3) KEWHL players follow the Policies & Procedures and Code of Conduct of the ASHN
4) KEWHL will supply a hockey sweater that must be returned at the end of the season
5) Registration refunds may be requested before October 1, 2019. For health reasons only refunds may be requested prior to November 1st, 2019. A $100.00 non-refundable
administration fee applies to all refunds. NO REFUNDS AFTER NOVEMBER 1, 2019
6) There will be a $25 Charge for NSF Cheques
