1. Duly accomplished application form 1419 (must be printed in A4 size bond paper)- see

2. Form 956 (to fill out #14 only and sign #24) - see attached
3. Checklist for Tourist, must be signed by applicant - see attached
4. VFS Consent Form - see attached
5. 2 pcs. passport sized photos (MOST RECENT with White background w/o eye glasses)
6. Scanned Copy of Passport (Current Passport should be valid for more than 6 months)
7. Certificate of Employment
8. Business Registration (if owner)
9. School Certification (if student)
10. Bank Certificates (Original)
11. Income Tax Return (copy only)
12. Copy of Land Titles or Car Registration (if any)
13. Flight Itinerary
14. Hotel Booking
15. Invitation Letter (if applicable)

For visa application with sponsor in Australia, additional docs needed:

1. Affidavit of support from the sponsor
2. Copy of Passport and Visa of the sponsor
3. Copy of Citizenship & Permanent ID of the sponsor
4. Employment Certificate of the sponsor
5. Copy of Land Registry of the sponsor
6. Bank Statements of the sponsor

Additional Documents for accompanying dependents:

1. Letter of Support from principal applicant
2. Copy of Marriage Certificate for dependent spouse
3. Copy of Birth Certificate for dependent child
4. School Certification for dependent child
5. Copy of NSO Birth Certificate of applicant traveling with dependent parent/s

Additional Requirements for Foreign Nationals:

1. Copy of I-CARD or AEP
