1 Mayor's permit issued by the city or municipality where the principal place of business of the prospective bidder is located, or the
equivalent document for Exclusive Economic Zones or Areas.

In cases of recently expired Mayor’s/Business permits, it shall be accepted together with the official receipt as proof that the bidder
has applied for renewal within the period prescribed by the concerned local government unit, provided that the renewed permit
shall be submitted as a postqualification requirement in accordance with Section 34.2 of this IRR.

3 Latest Income or Business Tax Return

4 Sworn Statement in accordance with Section 25(b)(iv) of the IRR of RA 9184 using the prescribed Forms

Note: Procuring Entities already maintaining an updated file of any of the bidder’s abovementioned
requirements, whether through the PhilGEPS Certificate of Registration and Membership or its own records,
may no longer require its re-submission.
