BPI CHECKLIST OF REQUIREMENTS

Basic Requirements:
 Duly accomplished application form
 Duly accomplished MRI Form
 Marriage Certificate if married
 Certificate of No Marriage if single
- Legal separation papers / Annulment papers if separated
- Copy of 2 Valid ID’s with visible photos & signatures, also with 3 specimen signatures
- Latest Residence Certificate
- 1 2x2 picture

If Locally Employed:
- Certificate of employment (COE) indicating salary, position and length of service
- 3 months latest pay slips or latest ITR or 3 months latest bank statements where salary is
credited

If OFW - Seafarer:
 Latest Contract and Certificate of Employment from POEA
 3 months payslips and proof of allotment

If OFW Land-based
- Certificate of Employment (COE) with email address of supervisor or HR
- 3 months latest pay slips or 3 months latest bank statements where salary is
credited

If Self-employed - Sole Proprietor: (should be Corporation)

- Articles of Incorporation/By-laws/SEC Registration
- Latest General Information Sheet
- Latest ITR of business with attached Audited Financial
- 3 mos latest bank statements of business
- List of Trade references (at least 3 names w/ telephone nos. with contact person of major suppliers/customers)

If Self-employed – Corporation: (should be sole prop.)

- DTI registration & mayor's Business Permit
- Latest ITR of business
- 3 mos latest bank statements of business
- List of trade references (at least 3 names w/ telephone nos. with contact person of major suppliers/customers)

If Self-employed - Outside the Country):

- Business papers
- Latest 1040 & W-2 & 3 mos latest bank statements of business

If Practicing Doctor:
- Clinic address/es and schedules
- 3 mos latest bank statements
- Latest ITR

If Salespersons:
 3 mos latest vouchers or Bank Statements
 Latest ITR

If Rental of Properties:
- Rental/Lease contracts (indicating name of tenants and rental amounts w/ complete addresses
of properties being rented)
 Photocopy of Title (TCT/CCT)
 Latest Income Tax Return if possible with attached AFS and bank statements

If Pensioners
- Pension papers
- 3 mos latest bank statements where pension is being credited

If Priests:
- Certificate from the Archdiocese

Collateral Documents (not accredited):

- Photocopy of Owner’s Duplicate of TCT/CCT (2 copies)
- Photocopy of Tax declaration on lot & building
- Lot Plan / Vicinity Map
- Non-refundable Appraisal Fee of P 5,000.00 (to be deposited to 1021-0208-81 BPI Family
Savings Bank Inc.

- Statement of Account from developer / Term Sheet

- Photocopy of individual TCT/CCT (2 copies)
- Photocopy of Tax declaration on lot & building

- 5 sets of notarized SPA (if still here in the Phils; should be notarized before leaving)

- 5 sets of Transferee's Affidavit (if still here in the Phils; should be notarized before leaving)

- Copy of Oath of Allegiance (for dual citizens)
