Greetings!

I am writing to request financial assistance for my hospital bills during my

hospitalization. That the undersigned is a bona fide member of Philippine National
Police and currently assigned in Regional Mobile Force Battalion 7, 704th Maneuver
Company located in Poblacion Mabinay Negros Oriental.

That on January 11, 2018 at around 3:00 PM, with the supervision of our Force
Commander, we left from our station/704th (RMFC) and went to Sitio Manganay,
Barangay Panalaan, Bais City, Negros Oriental Purposely to conduct surveillance on
the wanted/convicted persons for Homicide with Criminal Case No. 08-78-B, (issued
and signed by Hon. CANDELARIO GUILLERMO V. GONZALEZ, of RTC, Branch 45,
Bais City). The subject persons are identified as JUVY SABARICOS, male, of legal age,
and his brother MARLON SABARICOS and FELOMINO SABARICOS, of legal ages, all
residents of Panalaan, Bais City, Negros Oriental. Upon our arrival at the place of our
targets, we saw three (3) suspicious persons.

That later, our covert informant confirmed to us that the two (2) persons were our
targets. At that juncture we discreetly drew near to one of the houses of the suspects
and identified ourselves as police officers. Our team leader courteously conversed and
convinced the wife of one of the wanted/convicted persons to yield upon us as we had
the Warrant of Arrest against her husband. However, the wife just answered to us in the
negative. Unknowingly, the three male persons positively identified by our clandestine
informant as the wanted/convicted persons, sneaked out and hid themselves in the
sugarcane plantation located at the back of the house nearby, without any warning the
malefactors concertedly fired toward us, with the use of improvised/homemade
shotguns, thus despite the fact that we are already Wounded In-action (WIA) in the first
burst of enemy fire, we still struggled and forced ourselves to return fired at them.

That the firefight ensued between us and the wanted/convicted persons lasted for
twenty (20) minutes more or less. Considering the unforeseen incident and the
suspect’s familiarity of the terrain, I PO1 Relebert Berionio sustained multiple gunshot
wounds in the different parts of my body while one of my comrades was hit in his upper
left shoulder. Thereafter, the subjects were able to skedaddle and flee inside the
sugarcane plantation. That right after I was then brought to Bais District Hospital for
immediate medical attention. Later, being referred to the Siliman University Medical
Center (SUMC), Dumaguete City for further medical attention. That due to financial
constraint I am requesting to your good office for financial assistance.
