ICT DGTE Amazing Race 2019

COMPANY Volunteers WAIVER, RELEASE OF LIABILITY, INDEMNIFICATION, AND ROSTER CHANGE FORM
The undersigned Company Volunteer states and agrees as follows:

1.) The undersigned agrees that Team Member voluntarily participates in the Amazing Race and assumes all risks associated with such activity.

2.) The undersigned acknowledges that there are risks and hazards in participating in the Amazing Race including, but not limited to, those caused by Site/Stop
Conditions, playing conditions, equipment and other participants, in addition to the acts of having to roam the City locating the next Stop and performing all
the various Challenges at each Stop with other Team Members, spectators, or stationary objects, any of which may Cause death or serious injury to the Team
Member.

3.) In consideration of sponsor ICT DGTE and the co-sponsors CodingDepartment, Inspiro, and SMART allowing us to participate in this activity, I AGREE TO
INDEMNIFY AND HOLD HARMLESS said sponsors, its officials, employees, agents and assigns from any and all claims, demands, lawsuits, costs and attorney’s
fees arising out of participating in this activity. This Agreement shall be effective and bind any and all heirs, dependents, executors, and assigns.

4.) I ACKNOWLEDGE THAT I HAVE THE RIGHT AND OBLIGATION TO INSPECT the Stop and areas surrounding prior to each challenge and agree to play in the “as
is” condition. If I find the Stop or the Challenge is not safe, I have the right to choose not to participate.

5.) I acknowledge that I have read the foregoing instrument and understand its terms and agree to abide by its terms and conditions.
