Use this form to request approval from Gaming Licensing and Development Department (GLDD) for addition, conversion or
removal of a game offering. Addition refers to introduction of a new game offering at the gaming site. Removal of game
GLDD Reference No.: ____________
offering refers to cessation of operation of a current game offering. Conversion of game offering refers to removal of a game
offering from the gaming site for replacement with a new game offering. Please accomplish this form by providing correct
and complete information. The Operator shall submit this form to GLDD at least fifteen (15) business days prior to
intended date of implementation. Cut-off for submission of requests shall be 4:00 P.M.

We undertake to comply with the following guidelines on game offering under Regulation 7 of the Gaming
Site Regulatory Manual and post the required Performance Cash Deposit and Betting Credit Fund (for
Electronic Games) for the new game offering prior to its commencement of operation:
(a) For introduction of game offering:
 Introduction of new game offering at the gaming site must comply with the distance restriction to
another gaming site with the same game offering.
 New game offering shall be deployed at the existing gaming site or in the same building where the
existing gaming site is located.
 Should the introduction or removal of game offering require temporary closure of the gaming site,
the Operator shall submit the duly accomplished Temporary Suspension of Operations Form (GS
Form No. 6) for approval.
 If suspension affects some gaming terminals, a project timetable is attached which includes the
schedule of gaming terminal suspension in the affected areas.
 The Operator shall conduct a pre-operational testing on newly installed electronic gaming system/s,
network connectivity, electronic gaming terminals or machines and other gaming equipment, and
shall ensure their readiness prior to gaming operation.
 The Operator shall notify GLDD of the readiness of the gaming system and machines/terminals for
the conduct of pre-operational testing.
 The installation of the electronic gaming/betting/bingo terminals/machines into operation shall be
closely coordinated with Compliance Monitoring and Enforcement Department (CMED) and its
representative/s.

 The Operator shall submit a copy of amended Business Permit of the existing gaming site to cover
the new game offering.

(b) For removal of game offering:

 Upon acknowledgment by GLDD of the form, the Operator may proceed with removal of game
offering. Signage, electronic gaming/bingo machines, gaming equipment and marketing collaterals
associated with the removed game offering must be removed, in accordance with Regulation 15 of
Gaming Site Regulatory Manuals.
 The Operator shall agree that its outstanding monetary obligations to PAGCOR incurred from
operating the removed game offering shall be deducted from its Performance Cash Deposit (PCD).
Refund of the PCD, for the ceased game offering shall be in accordance with the guidelines under
Regulation 4 of the Gaming Site Regulatory Manual.
(c) This also certifies that the following documentary requirements are attached:
 Existing gaming site layout  Proposed revisions to CCTV placements, if any

 Proposed gaming site layout  Project timetable, as applicable

 Copy of valid Business Permit  Copy of O.R. for paid Performance Cash Deposit
