RECOMMENDATION FORM

INSTRUCTIONS
This recommendation form should be answered by a person of authority who has worked with the applicant. The
recommendation should focus on highlighting the applicant’s leadership potential and his/her commitment to work
in the field of social development.
All application documents including the recommendation letter should be sent on or before 11 September 2019
(Wednesday) at 11:59 PM. Please inform the person giving the recommendation to send this letter directly to FES:
via fax to (632) 632-0697 or email it to reboot@fes-philippines.org. They may also seal it in an envelope and (1)
give it to the applicant directly for submission, or (2) mail it to: Unit 2804 Discovery Centre, #25 ADB Avenue,
Ortigas Center, 1605 Pasig City, Metro Manila.
Please use “Recommendation for (Applicant’s Name)” as the subject in emailing the recommendation form.
A confirmation email will be sent to both applicant and the person giving the recommendation to inform that this
form was received.

 5. REBOOT is an intensive ten-month training program that will require the participants to immerse in
marginalized communities, create projects with community organization partners, and collaborate remotely
with 35 colleagues across the Philippines.
