The undersigned was designated to conduct an investigation of alleged series of theft of

umbrellas in the Ateneo de Naga University Library. The initial information that served as a
basis for this investigation was provided by University Security Guard Joseph Manalo [Identify
person or persons who supplied information of possible misconduct] and consisted of his written
statement attached herein.

The investigation was initiated on October 3, 2018 and concluded on November 15 2018.

All potential witnesses that have been interviewed and documents that were reviewed are
listed as follows:

 Record of entries in the university library

 Student Maria Chua Cheng (20 year old, 4th year college student)
 Student Keiko Ferrer (20 year old, 4th year college student)
 Security Guard Joseph Manalo
 CCTV footages in the library
 Security Guard Bobby Sanchez

Findings of Fact

- For the first semester SY 2018-2019, Anne O. Nadeer, has been frequently sighted by
Security Guard Joseph Manalo and Bobby Sanchez to be often visiting the University
Library.

- On August 2, 2018, Student Maria Chua Cheng lost her umbrella in the Library’s
umbrella rack while she was studying inside. She then reported the incident to the
Security Guard Joseph Manalo.
 - Security Guard Joseph Manalo who happens to be on duty that said day have seen
Student Anne O. Nadeer took 2 umbrellas on the rack that day, in her defense that her
friend asked her to get it for her.

- On September 15, 2018, Student Keiko Ferrer also happens to lost her umbrella on the
same premise where Student Maria lost hers. She reported the incident to the Security
Guard on duty, Bobby Sanchez.

- Security Guard Bobby Sanchez said that he saw a girl (who was later on identified as
Anne O. Nadeer) who took an umbrella which happens to have similarities of the said
lost umbrella, as Student Keiko Ferrer described it.

- CCTV footages were obtained to resolve the matter, Student Anne O. Nadeer has been
identified and confirmed to be the primary suspect in the events of the losses of the
umbrellas at the University Library.

Opinions

- It is clear and positive that Anne O. Nadeer is the culprit behind the series of losses,
evidences and testimonies have been established against her.

Recommendation: Based upon the evidence that has been collected, it is recommended that the
OSA director considers a recommendation of disciplinary action, and or sanction against Nadeer
and any possible accomplice, to the Board.
