AFFIDAVIT

I, VERONICA LOPEZ PEREZ, of legal age, Filipino,

married and presently residing at Brgy. Sampaga, Greenhills,
Batangas City, after having been duly sworn to in
accordance with law hereby deposes and states:

That I am married to RODELIO DE CASTRO PEREZ;

That however, I have been separated from my said

husband for more than two (2) years now;

That in the course of our marriage, we have been

blessed by three children, namely: Nikki Dave (19 years
old), Coleen (14), and Kazsandra (10);

That I have the sole custody of our three children;

That as such, I am the only one taking care of and

providing for our children;

That I have become a solo parent since the time we

have separated because I have not received any help from
my husband;

That I am executing this affidavit to attest to the truth

of all the foregoing facts and circumstances and for
whatever legal purpose this may serve.
