OPERATION IN MARANATHA CHRISTIAN ACADEMY OF MANDALUYONG CITY, INC.
ON AUGUST 17, SATURDAY.

Dear Hon. Kap. Castillo,

This refers to the Dengue Prevention and Control Program of the City spearheaded by the City
Health Office through the Environmental Health and Sanitation Section. Regular Fogging and Misting
operation has been conducted has been conducted yearly on the said institution.

However, due to some incidence of Dengue in the City and in order to contain the Dengue Vector
Mosquito, the City Health Office or in respective barangay will conduct Fogging and Misting Operation
yearly both public and private to protect our students, faculty, and staff from this contagious illness.

Wherefore, we are requesting your utmost cooperation on this activity, as herein enclosed the
proposed schedule on August 17, Saturday from 8:00 in the morning onwards. Further, prior to this activity
a massive promotion of KAIZEN 5S this includes Sort, Systematize, Sanitize, Standardize and Self
Discipline in our institution to eliminate probable harborage of mosquitos. Thus, any clogged drainage or
with stagnant water cases should be reported immediately to the School Repair and Management Group
for their immediate action.

Therefore, prior to this operation we would like to request your assistance for the effectiveness of
this operation.
