1. Relative to DepEd Memorandum dated March 7, 2019, entitled Establishment of Database of

Learners per Job Group for Various NEAP Learning and Development Programs, this
Division requires the school heads and other concerned personnel to prepare and to submit the
needed data on March 21, 2019.

2. All schools must submit the Learning and Development Profile of their personnel using the
template that may be secured from these links:

3. Download the file as an Excel File by clicking "File" and "Download as Excel", fill out
with data requested in the form. Name entry must be in alphabetical order in the ascending
order of designations (Ex: All the names with Teacher I designation should be entered first in

March 21, 2019.

6. All Division Chiefs, Public Schools District Supervisors, Education Program Supervisors, and
Senior Education Program Specialists must accomplish the online form on the link right
