Warning letter

I am writing to you about your recent absenteeism from work, and specifically the handling of
reporting such time off work to Management.

On Monday of this week 3rd July 2017, you were absent from work presumably off sick. As with
normal procedure and in line with Company policy, it is mandatory, unless in extenuating
circumstance, the employee contacts his/her Manager notifying them as soon as possible of the
situation. Furthermore, in accordance with current legislation as per the ‘fairwork act’ employees are
to provide proof of sickness.

This letter is to advise that the correct procedure was not followed and that your recent conduct has
been unsatisfactory, and that immediate improvement is required.

After considering the situation it is expected that your conduct improves and specifically that you
follow the company policy and procedure mentioned above also aligned with the current statutory
governed legislative law of the ‘fairwork act’.

This is your first warning letter and you have been put on notice. Please be advised that repeated
performances will result in further warning(s) and your employment may be terminated if your conduct
does not improve.
