La analgesia postoperatoria adecuada, especialmente después de una cirugía abdominal mayor, es importante para la
recuperación, la movilidad temprana y la satisfacción del paciente. Nuestro objetivo fue estudiar los efectos de la crioterapia
a través de una bolsa de hielo en el postoperatorio inmediato, para pacientes sometidos a operaciones abdominales mayores.

Métodos

Este estudio prospectivo se realizó en nuestro centro de referencia de atención terciaria en un país de ingresos bajos y
medianos. Las características preoperatorias del paciente, las variables intraoperatorias y los resultados postoperatorios se
compararon entre dos conjuntos de pacientes. La crioterapia se administró a través de paquetes de gel congelado durante 24
h inmediatamente después de la laparotomía. El alivio del dolor se evaluó con puntajes analógicos visuales de dolor (EVA).
Las comparaciones entre grupos se midieron mediante la prueba de Chi-cuadrado, la prueba exacta de Fischer o la prueba U
de Mann-Whitney , según corresponda.

Resultados

Sesenta y ocho pacientes fueron incluidos en el estudio: 33 en el grupo de crioterapia y 35 en el grupo sin crioterapia. Las
puntuaciones medias de dolor postoperatorio (EAV) fueron significativamente más bajas en el grupo de crioterapia frente al
grupo control (3,97 ± 0,6 frente a 4,9 ± 0,7 en el día postoperatorio (POD) 1; p <0,001 y 3 ± 0,5 frente a 09 ± 0,8 en POD2 ;
p <0,001). La mediana del uso de narcóticos en equivalentes de morfina fue menor en el grupo de crioterapia de POD 1-3
(66 (IQR-16) frente a 89 (IQR-17); p = 0.001). No se observaron diferencias significativas en el uso de AINE entre los
grupos. También se descubrió que el grupo de crioterapia tenía una menor incidencia de infección del sitio quirúrgico ( p =
0.03) y una mejor función pulmonar basada en la espirometría de incentivo ( p = 0.01) y demostró una recuperación
funcional anterior basada en su capacidad para realizar la prueba de estar sentado ( p = 0.001).

Conclusión

Las bolsas de hielo son un adyuvante simple y rentable para el tratamiento del dolor postoperatorio estándar que reduce el
dolor y el uso de narcóticos y promueve la rehabilitación temprana.
