Re-FAT carried out on 07.08.2018:
30 3 23 34PCV -045 4" Flanged, 300 lb, RF, 125-250 AARH 900 1. During Fail safe (FO, Air supply failure) test, it was observed in Positioner, the max. lift of stem goes till 116.7% (17.5 mm travel), instead of
100%. Positioner to be configured so as to meet 100% (±0.5% accuracy) of max. lift. now currently it reads 102.7% at max. lift - closed 0n

Test carried out on 01.08.2018:


1. Being TSO valve, there is 5 bub/min leakage as against 57 bub/min.-Closed

4. 1/4" tubing used between VB to Positioner outlet. Other tubings are 1/2". Pneumatic scheme drawings to be updated by supplier.

7. During Fail safe (FO, Air supply failure) test, it was observed in Positioner, the max. lift of stem goes till 122.5%, instead of 100%. Positioner to

8. Flow characteristics to be updated with "Mod. EQ%" in the Name plate detail & supplier spec sheet as per GAD & Sizing sheet.-Closed.

Test carried out on 31.07.2018:


1. Being TSO valve, there is 10 bub/min leakage as against 57 bub/min.-Closed
2. During Fail safe (FO, Air supply failure) test, it was observed in Positioner, the max. lift of stem goes till 118.0%, instead of 100%. Positioner to
be configured so as to meet 100% (±0.5% accuracy) of max. lift.- now currently it reads 101% at max. lift - closed 0n 31.10.2018 ,
3. 1/4" tubing used between VB to Positioner outlet. Other tubings are 1/2". Pneumatic scheme drawings to be updated by supplier.

be configured so as to meet 100% (±0.5% accuracy) of max. lift.-now currently it reads 100.2% at max. lift - closed 0n 30.10.2018.
3. Painting color peeled off at some portions of the valve flanges and actuator.-closed.

Test carried out August:

Re-FAT carried out on 06.08.2018:


1. Leakage observed in Packing area, even after the packing was adjusted several times. Supplier to carried out FE test after adjusting the
packing.- Closed on 30.10.2018, 1. Samson AG reoffered the valve after rectification Body mount there is no leakage observed
witnessed and found satisfactory. 2. Seat leakage Test carried out and found within the limit accepted as per leakage Class IV. 3.

FAT test carried out Test results were found satisfactory. 4. Wired Tag Plate provided Valve & RV by Samson AG witnessed by

this TAG - Closed.
2. Painting on Bonnet flange is peeled off on some portions. Supplier to verify and resolve- Closed on 05.11.2018

to be configured to meet 100% of max lift. Closed on 01.11.2018 Samson AG reoffered the valve and (FO, Air Supply failure) test
witnessed and found lift stem goes up to 103 % found within the limit.
