CONTENTS
Safe Work Procedure (Safe Work Procedure for Electrical Safety) is designed to
minimize risk, to prevent incidents through promotion of safe work practices and
behaviours during the execution of the activities for inside the BLOCK S2 area,
Republic of YEMEN.
This procedure is to establish minimum requirements and guidelines to provide
the maximum prevention protection against burns, electrocution, electrical fires,
explosions, electric shocks, etc. These guidelines detail the necessary
precautions that should be taken by all personnel to minimize their exposure to
electrical sources of energy.
Definitions
The following definitions shall apply in this specification.

connected to the electrical installation.
+ RCD Residual Current Device. A switching device
intended to isolate the circuit when the current imbalance attains the rated
operating leakage current value of the device
+ ELECTRICAL LOW RISK AREA: A working environment that is clean,
well organized and free of environmental factors that could affect the
‘equipment or flexible supply lead.

In case of conflict between the various codes, standards and the requisition, the
most stringent of the requirements shall generally govern. The SUPPLIER shall
refer any such conflict and any other matter which requires further interpretation
to the attention of COMPANY and COMPANY's interpretation, shall be final.
4, RESPONSIBILITY

+ Has the final responsibilty to ensuring that all personnel on the project shall
comply with the requirements of electrical works as herein mentioned in this
procedure.
+ Shall take appropriate disciplinary action for the violators. As per the
recommendation of HSE Manager.
42 Construction Manager
+ Provide the required protection for the personnel/crew involved in electrical
activities.
+ Apply for HSE training for personnel and ensure that the person attend the
necessary training provided by the CONTRACTOR HSE Department.
+ Ensure that electrical protection equipmentsidevices used are in good
condition and that it serves the intended purpose.
+ Ensures that all employees receive PPEs and sees to it that they use them
where necessary and maintain them correctly

+ Should ensure that all personnel shall wear the correct personal protective
‘equipment before they can perform electrical work activity,
+ Ensure HSE training for the personnel.

+ To ensure that all personnel are ; adequately trained for the relevant task;
that they are using the correct equipment and personal protective equipment;
that all relevant hazards have been identified; and that the respective permit

ELECTRICAL SAFETY
+ Must conduct a tool box talk meeting with his crews about the electrical work,
related hazards and control measures to prevent any kind of injury.
+ Has responsibilities of providing the required PPEs to his crew for the job as
mentioned herein this procedure.
+ Housekeeping is done at the end of the day and the waste to be disposed off
as —_ per project waste disposal procedure.
+ Responsible for ensuring all electrical works are performed in a safe and
proper manner and that the equipments are left in a safe condition.
+ Ensures that his personal competence, previous experience and training on
specific works in electrical and instruments adequately serve the
requirements of the job to carry out safely
ELECTRICAL SAFETY INDUCTION
Electrical Safety Induction shall be applicable to all contracto’s personnel
‘engaged in Performance of constructiory installation, inspection, testing, pre-
commissioning, commissioning, activities to be carried out for the execution
of the scope of work at the ‘live Plants" facilities.
+ This Electrical Safety Induction shall be performed by the “Live Plants"
‘operation groups in order to introduce the plants Safety Requirements, which
CONTRACTOR'S personnel shall comply with in execution of all activities.
+ In addition the CONTRACTOR'S personnel shall be trained to obtain
Electrical Safety Accreditation level i.e. Electrical Authorization.
+ Electrically Authorized Personnel and Senior Electrically Authorized person
and all personnel working on live equipments must follow up electrical safety
training from a third party (ie. APAVE ) and get accreditation from
CONTRACTOR and COMPANY. CONTRACTOR personnel not performing
work on live equipments but doing other activities at the close vicinity of the
live equipments must receive electrical safety training and get accreditation
from CONTRACTOR,
6. COMMON ELECTRICAL HAZARDS
+ Electric shock/electrocution occurs when current flows through and
damaging the body.
+ Electrical burns are caused by arc blast or hot conductors.
+ Indirect falls from ladders, scaffolds, or other walking and working surfaces,
+ Explosions can be caused when oloctricity provides a source of ignition for

+ Fires are caused by overloading a circuit or appliance or by current flowing
through high resistance due to faulty wiring, setting fire to insulation, and
surrounding materials.
GENERAL REQUIREMENTS
+ CONTRACTOR shall ensure that electrical equipment is free from
recognized hazards that are likely to cause death or serious physical harm to
employees. Safety of equipment shall be determined on the basis of the
following considerations:
= Suitability for installation
+ Mechanical strength and durability
* Electrical insulation
+ Heating effects under condition of use
+ Arcing effects
* Classification by type. size, voltage, current capacity, specific use.
+ Equipment shall be installed and used
accordance with instructions.
+ Splices
+ Splicing devices suitable for use
+ Welding/brazing/soldering
+ Mechanically/electrically secure before soldering
+ Covered with insulation equivalent to that of the conductors
+ Insulating device suitable for purpose
+ Each service, feeder, and branch circuit, at its disconnecting means or over
current device, shall be legibly marked to indicate its purpose.
+ Live parts of electric equipment operating shall be guarded against accidental
contact by cabinets or other forms of enclosures.
8. SEQUENCE OF PROCEDURE

1000 ohms), and current (e.g., 0.12 amperes). Of the three, current is what

ELECTRICAL SAFETY
+ During electrical equipment failures or circuit failures, electrical current can
use the human body as a path of least resistance. This can cause injury and
death. When using electrical equipment, anything that reduces the body's
resistance can significantly cause more current to flow through the body
during electrical failures,
+ When electrical current flows through the human body, it is the combination
of the magnitude of the current and the duration of that current that
determines the severity of the injury.

itenance Personnel Safety Requirements
Outlined below are requirements that shall be followed in an office environment
by apply equally as well at the cabin or other work environments.
+ Never handle electrical equipment or their extension cords when hands are
wet or if the floor surface is wet.
+ Always unplug equipment when performing maintenance. Equipment should
be unplugged by grasping the entire plug itself and not by pulling on the cord,
This applies to disconnecting a portable drill, changing a bit to disconnecting a
Portable saw to replace a blade etc.
+ Never use electrical equipment that has defective plugs or wiring. Cords that
are frayed or have insulation tears, cracks, breaks, of abrasions shall not be
used. Any repairs made to such defective-equipment shall be done by
qualified electricians. Three prong plugs shall be inspected from proper
orientation of the plug prior to inserting it into a receptacle.
+ During the initial installation of any equipment, the electrical cord shall be
visually inspected for defects. In addition, anytime the cord is moved from
fone location in that office to another location in that same office or other
locations, it shall be visually inspected again. It is the user's responsibilty to
visually inspect the equipment or extension cords.
83 Extension Cords
+ Be aware that all locations adhere to an “Assured Grounding Program”. This
program stipulates that portable tools, portable equipment and extension
cordsicord sets be tagged / coded to indicate when they require retesting
The user of such equipment and tools is responsible for ensuring that the
inspection dates are valid. If in doubt, seek assistance.
+ Extension cords in one office shall not be used to supply power to another
office, building or adjacent offices. Cords may not be run through doors,
windows or ceilings.

protected from traffic and abuse
+ Extension cords as well as equipment cords shall not be modified to fit a
receptacle. Specifically, ground prongs on cords shall not be modified,
removed or fitted with adapters.
+ Extension cords shall not be overloaded. Care shall be taken to ensure that
the cord has sufficient capacity to carry the electrical load. Indications of
overloads are breakers tripping, receptacles that are warm to the touch,
extension cords that are warm or the smell of electrical insulation burning,
+ Extension cords that are frayed or have insulation tears, cracks or abrasions,
or with bent, broken or ‘spread’ prongs shall not be utilized. Defective
prongs can be a source of shock or electrical fires.
Grounding
+ All equipment shall have an equipment grounding conductor that connects
the frame of the device being utiized to the electrical system grounding
conductor.
+The equipment ground is required to protect personnel. Equipment shall be
grounded or utilize a double insulated system. Ungrounded equipment and
tools represent a hazard to personnel.
+ Two prong plugs provided with double insulated equipment shall be installed
with the correct plug orientation.
NOTE: If in doubt, seek the advice of a qualified electrician or immediate
supervisor,
Plugs and Receptacles
+ Equipment shall not be unplugged while that equipment switch is turned on or
plugged into receptacles with equipment's switch turned on.
+ Special circuits protected by Ground Fault Circuit Interrupters (GFCI) are
used to provide added measures of protection to personnel. They are easily
identified by a ‘push to test” button and a “reset” button and are labelled
“GFCI". Any equipment that causes these receptacles to trip or deactivate the
circuit shall not be utilized unless inspected by an electrician. Do not attempt
to use such equipment on non-GFCI receptacles. Using a device that trips a
GFCI on a non-GFCI circuit can result in receiving an electrical shock or

86 Electrical Safety Requirements
+ Portable electrical hand tools must be grounded or double insulated.
+ All electrical cords and cables are covered or elevated to protect them from
damage and to eliminate tripping hazards, cables must be away from water.
NOTE: When cables are elevated they must be secured into position and
protected from abrasion when strung over metal beams, etc. The use of rebar tie-
wire to secure cables is prohibited.
+ Power saws, grinders, and other power tools must have proper guards in
place at alll times.
+ Temporary portable lighting used in damp and/or hazardous locations and
confined areas with low ground resistance must be operated at a maximum
of 12 volts, unless protected with GFCI's.
+ Qualified electricians are the only employees authorized to repair electrical
equipment. Field repairs or tampering with any electrical equipment by
unauthorized persons is not tolerated.
+ Distribution panels must be dead front type, covering hot terminals and
properly constructed and grounded.
+ High voltage electrical panels must be properly protected and identified using
approved signs.
+ Cord sets must have heavy duty insulation, weather and sun resistant and
free of splices.
+ Allelectrical equipment is to be visually checked by the user daily or before
each use. All electrical equipment is formally inspected by a qualified
electrician at least every 90 days (quarterly) or more often based on
conditions found during inspections. Such inspections include @ continuity
test of the grounding conductor (as applicable) and complete examination of
the equipmentisystem to assure good and safe operating condition. No
electrical equipment is used unless it is in proper operating order.
+ When performing maintenance on temporary electrical systems, lockouts and
tag outs must be used. Lock Out / Tag out is discussed in another Safe Work
Procedure (YE-KHA-00-094- 140812).Under no circumstances shall any
qualified electrician to perform adjustments, modifications, etc. on any
equipment, tools without the prior permission of the Concerned Department.
All authorized work will be subject to inspection at anytime.
+ During testing and commissioning processes of permanent plant equipment,
the lockout and tag out program established by the CONTRACTOR and
COMPANY must be complied with and under the control of the Safety

ELECTRICAL SAFETY
+ Only qualified electricians make electrical connections
+ Notify all personnel who may be affected when the power is tumed off, and
again before it is turned on,
+ Do not render interlocks unserviceable by removing or modifying them. They
are there for a purpose.
+ Know the voltages and frequencies of circuits before any work is done.
+ Know the capacity of fuses and circuit breakers.
+ Check all cords to all electric tools and all portable electrical equipment to
ensure they are serviceable and properly grounded,
+ Water must not be used on electrical equipment fires. Whenever possible, do-
energize electrical equipment before fighting the fire.
+ Extreme caution must be used when equipment is being operated near
power lines. The lines must be de-energized if possible.
+ Do not work on electrical equipment when clothing is wet or any part of the
body is in contact with water.
+ Do not wear rings, watches or carry other metal objects such as lighters or
keys when working on any electrical system.
+ Any equipment or structure, on which electric charges may accumulate, such
as storage tanks, must be grounded.
ELECTRICAL INSTALLATIONS

+ Cable trays or raceways shall not be used as a means of access or as a
substitute for ladders or work platforms.
+ Standing on or walking along cable trays are strictly not allowed unless they
appropriately are protected with 3/4” thick plywood (as minimum) or wood
planks.
+ Lanyard of full body harness shall not be hooked on to cable trays or
raceways as they cannot withstand the load in case of fall
+ Electrical / instrument cables being installed shall be appropriately protected
from possible source of damage such as hot welding / cutting splatters, hot
surfaces, open flames, sharp edges, chemical splash, mobile equipment and
the like,
+ Should the cable pulling / laying associated with the perimeter fence work
affect public traffic, a designated traffic man must be appointed and all

TREATMENT OF ELECTRIC SHOCK VICTIMS
The first priority for the rescuer is to avoid of becoming a casualty and making the
situation worse for other rescuers. It is essential to make sure it is safe to
approach and to check if the casualty is stil connected to the electrical current in
any way directly (to a faulty machine, apparatus, or to a bare wire), or indirectly
(lying on an electrically conductive surface).
+ Adequate provisions being made for calling the emergency services, refer to
Safety and Emergency Response Manual (TY-043-HSE-PLN-004) for the
Emergency Contact List
+ Workers who are expected to help in the event of an incident being instructed
on the procedure to follow to isolate the supply
+ Concern personnel on site being given practical training in techniques for
treatment in the event of electric shock

Site Manager
The CONTRACTOR Site Manager is responsible for the overall implementation
and coordination of the procedure. He shall make all the necessary arrangements
to ensure full compliance to the required codes, standards, procedures and
regulations. The site manager is required, as far as reasonably practicable, to
ensure that the company's codes and standards are met and implemented within
his organization.
Project Manager
Responsible person in the implementation of the HSE Audit and Inspection
Procedure. He shall create an audit team through CONTRACTOR HSE Manager
that will make sure that all the provision of this procedure is followed. The Project
Manager shall make sure that this procedure is reviewed and updated based on
the current Regulations or Procedures set up by the COMPANY.

+ Apply for HSE training for personnel and ensure that the person attend the
necessary training provided by the CONTRACTOR HSE Department,
+ Ensure that the work site and tools and equipments used are in good
condition and that it serves the intended purpose.

+ Must ensure that corrective action is taken against infringements as they
apply to their respective areas of accountability and ensure that follow-up
inspections are performed. Each position is also responsible to participate in
recognition programmes, which may be implemented for HSE achievement
during the course of the project.
+ Participation in all auditing processes, along with interaction with COMPANY
and follow-up action falls within the scope of site HSE procedure.
Accountability for HSE performance is measured through results of the
auditing programme as well as follow-up action.
+ Construction Managers are also responsible to render prompt corrective
measures to all aucit items and awards programmes. Pro-active approaches
by subcontractor to identify and correct concerns, as an organization, are
expected.
INSPECTION AND AUDIT PROGRAMME
Daily Inspection
Routine & continuous inspections are performed throughout the work-day. These
inspections are unannounced and random. These inspections are not normally
documented but involve a direct dialog between the inspector and the field
supervisor and/or managers as applicable to the situation. Observations are
discussed along with corrective action be taken. Any unsafe acts or situations:
were recorded on the “USOC” un-safe observation card. Recommendations and
close out actions followed up by HSE personnel
The HSE officers / CONTRACTOR watchers may find it necessary to document
items they feel important. Other means to document concerns are formal
correspondence & discussions at the construction meeting where minutes of the
meeting are taken.
CONTRACTOR HSE personnel will conduct continuous daily safety inspections at
work site and, where required, prepare and issue to the responsible Foreman /
Supervisor a "USOC" card for necessary corrective actions on observed safety
hazards, unsafe condition or practices. Copies of issued Safety Violation / USOC
Card shall be kept on file at CONTRACTOR HSE Office.
CONTRACTOR shall perform technical and safety reviews for any part of the
work and or the plant and shall provide COMPANY with the corresponding reports
upon request.
CONTRACTOR believes that the only way to achieve an incident and injury free
workplace is to utilize behavioural safety techniques (observation program) to

Weekly Walkthrough
Every week a management HSE walkthrough is performed at pre-selected areas
specified by the CONTRACTOR HSE manager. The inspection is performed by a
team consisting of:
+ COMPANY HSE Representative
+ COMPANY Construction Representative
+ CONTRACTOR HSE Representatives
+ CONTRACTOR Construction Representatives
Notes are taken by the CONTRACTOR HSE Representative as observations are
made during the walkthrough. Positive items as well as opportunities for
improvement are recorded. Review of actions taken with regards to the last
walkthrough should be also performed. A discussion of the inspection is
performed by the leader of the team at the conclusion of the walkthrough.
Weekly Inspection
Any deficiencies noted during this inspection shall be immediately addressed to
the responsible Supervisor or Foreman for prompt corrective measures. The
safety officers shall make follow-ups on the required action.
Any deficiency's found on items inspected shall be documented and so with the
corrective actions taken
Monthly Inspection
Monthly Safety Inspection will be conducted by the designated competent persons
(i.e. competent electrician for portable power tools, competent mechanic for
Equipment inspection, HSE personnel for safety devices, etc.) All monthly
inspections shall be documented and appropriate records maintained at HSE
department.
Monthly inspection for the following items shall be observed:
+ Lifting tackles (slings, chain hoists, come-along, shackles, etc.)
+ Gas cutting apparatus (flash back arrestors, hoses, etc.)
+ Portable electrical tools, extension cords, and other such equipment (fixed

Premises and Housing Inspection
CONTRACTOR shall inspect on monthly basis living quarters, camps, catering
premises, and offices.
Health and hygiene inspection of any accommodation and catering facilities
based on the COMPANY checklist shall be performed and followed up,

Measurement, monitoring and evaluation are key issues of HSE management.
The audit process is intended to ensure that all activities are being conducted in
accordance with established policies and procedures, and to identify areas where
there is an opportunity to improve performance.
To ensure a focused approach to project reviews, CONTRACTOR has
incorporated the use of formal auditing programmes.
Audits are conducted during the Project execution to ensure that:
+ All project HSE requirements have been incorporated into the Project HSE
Plan
+ Project procedures have been established which incorporate HSE
requirements
+ All scheduled reviews have been carried out as planned and
recommendations have been incorporated

recommendations will be tracked to completion in an action tracking system
maintained by the CONTRACTOR HSE Manager.
HSE studies and audit reviews that will be performed during the Project
‘execution shall be advised to COMPANY.

Corporate Audit
The project is subject to audits that may be held by corporate representatives
throughout the duration of the project. CONTRACTOR HSE program is evaluated
with 2 written report forwarded to the executive project manager. Replies to the
audit report are required. Corrective actions necessary to be implemented
Special Audit
There will be occasions where particular activities/conditions warrant extra-
ordinary review such as Tool Box Talk surveys, camp surveys, toolbox meeting
audits, cranes & equipment, fire prevention, etc. These audits normally involve
written reports issued from the CONTRACTOR HSE manager to departments
concerned.
THIRD PARTY CERTIFICATION
CONTRACTOR shall maintain available on the worksite for the duration of the
Contract valid third party inspection and test certificates,

Concerns identified during the auditinspection will be discussed as they are

Positive items should also be discussed and encouraged.
It is expected that corrective and preventative actions agreed will be initiated
immediately. Where completion of the action cannot be observed during the
inspection a completion date will be agreed.
Records of the corrective actions to be maintained in the CONTRACTOR HSE
Office. All corrective actions should be addressed to the pertinent department.

used on the site have been inspected and are safe to use. Instructs the
Supervisor to raise necessary permit for the hot work activity

The HSE Manager is responsible to monitor the implementation of this procedure

activities are in good condition and personnel are fully trained and aware of the
safe use.

All crews have adequate training and experience to work with the hand and
machine tools. Ensure necessary work permits to be obtained before starting the

preventive actions on their daily activities,

PREVENTION OF INJURY
Many hazards are associated with welding and cutting, but as in any other
operations they can be controlled by appropriate preventive measures. Unless
these measures are taken, the hazardous situations can easily give rise to
personnel injury andior material damage.

Eye Injuries
Eye injuries can occur as a result of exposure to radiant energy emitted during arc
welding or when plasma or flame cutting operations are undertaken.
Prevention from this of injury can be achieved by the person carrying out the
welding or cutting operation by wearing eye protection in the form of full face
shield filter with the appropriate dark lenses for welding and in the case of cutting
For personnel who are working within the area that may be affected by the
welding or cutting activities, welding screens will be erected around the welding or
cutting activity to effectively shield, those working close by from flash burns.
All such screens will be manufactured from a suitable fire retardant material and
should be erected in such a manner as not to restrict ventilation of the working
area,
It should also be noted that all personnel will be required to wear, as a minimum.
for general site work, safety glasses fitted with side shield. Damage to the eye
can also occur from the ingress of hot slag. sparks or flying debris caused by
chipping, cleaning-up or grinding of a weld. At all times while performing these
activities, it shall be mandatory for additional eye and face protection to be worn,

Burns
+ Thermal: Burns of the skin from hot metal, spattered slag or metal, or from
handling hot objects such as tools, rods or the work may be effectively
prevented by the use of protective clothing such as coverall, aprons, shirts
and gauntlets. Waring signs or barricades shall be considered to avoid
injury to another worker such as burn injury by touching welded parts in a
congested or busy area after welding
+ Radiant: Risk of bums from radiant energy to the unprotected skin of the
welder (Such as arms, neck, ears or face) is easily prevented by the wearing
of protective clothing.
Note: Clothing made from standard grades of nylon and other man-made fibers is
not recommended for protection against thermal or radiant burns.
+ All personnel directly involved in welding and cutting shall wear suitable
protective clothing, in addition to safety boots, helmets, etc. Such clothing
Shall include but not be limited to:
= leather gauntlets / gloves
= welders coverall.
Prevention of Electric Shock
+ All portable welding machines and generators will be positively earthed by an
ear thing stake manufactured from suitable material (copper bonded ground
rod) driven into the earth
+ All welding earths will be connected as close as possible to the workplace or
area where the welding is taking place.
+ Welding earths shall not, under any circumstances be connected to any
scaffolding, scaffold component or rigging equipment.
+ Welding cables shall be kept dry and free from grease and oil to prevent
breakdown of insulation. Cables with exposed bare conductors shall be
replaced or discarded immediately,
+ Welding cables shall, where practicable, be one continuous length. Should it
be necessary to connect two or more cables, only purpose made, approved
connectors shall be used for this purpose; pipes, steel bars shall never be
used.
Toxic Fumes and Respiratory Protection
Welding, cutting and brazing operations produce mixtures of gases, fumes and

metal, welding temperature, type of electrode and flux, metal covering,
contamination, shielding gases, etc.
Other gases, fumes and smoke are produced from the burning of the base metal,
welding rods, metal coatings, containments (e.g. solvents), fluxes, etc. The fumes
consists mainly of iron oxide but oxides of zinc, manganese, nickel, antimony,
copper, cadmium, etc, can be present, as well as fluorides.
Metal surfaces may be coated with protective films of lead paint, zine galvanizing,
cadmium, cil or plastic. When these surfaces are heated other toxic fumes are
generated.
To ensure protection of the person carrying out the welding or cutting operation
from inhalation of toxic fumes or vapors adequate open top welding booths to be
erected for exhaust. Or in situations that open top welding booth is not
practicable, exhaust ventilation shall be used. From time to time, a welder will go
out of the booth to breathe fresh air.
NOTE: Welding or cutting in enclosed spaces such as tanks can seriously deplete
the oxygen content of the air. Adequate ventilation is essential, supported by the
welder wearing an ait- supplied respirator when necessary. For works inside the
confined space, refer to TY-043-SWP-PRC-008 (Safe Work Procedure for
Confined Space Entry).
Oxygen Enrichment
Oxygen enrichment gives no warning; itis tasteless, odorless and colorless. It can
bring on a feeling of Euphoria. An oxygen enriched situation can easily catch fire;
textile can burn fiercely without even noticing any signs of danger. Pressurized
‘oxygen is not for blowing dust or cleaning clothing. Oxygen can saturate the
material making 8 times more flammable. See below the effects of oxygen
contents in the air.

Electric Shock Precautions
Good electrical connection, Electrode holder and cable properly insulated
Welding machines shall be equipped with ELCB to protect the welder from
sudden electrical failures of the welding machine from electric shock /
‘electrocution and welding machines to be inspected and calibrated by a qualified
electrician.
All electrical connections to welders, power supply to electrode holder clamps and
earthing connection must be made only with the circuit open.
Cables and connectors shall be capable of handling the maximum possible current.
The earth wire must be connected as close as possible to the element to be welded
using pliers, clamps or other systems which guarantee good contact.
Grounds shall not be connected to handrails, stairs, or from steel power or lighting
towers, or on any active oil, gas, steam, air, or chemical line: in live plant area ask to
plant-owner for suitable ground point.
Ground machine frames: inspect all grounds for mechanical strength adequacy.
Insulate welder from work piece and ground, particularly from wet area
Wear dry, hole free gloves.
Don't touch electrically “hot” parts or electrode or electrically live parts.
Always inspect the electrode holder before turning the welder on.
When working at height, use a safety harness
Housekeeping
Housekeeping shall be maintained in all areas especially where welding is taking
place. All welding stubs, used grinding disos, etc, will be contained in a receptacle
or metal bins provided by Supervisor/Foreman in the work activities, and located
at the work place. All such receptacles or metal bins shall be emptied daily and to
be dumped in the designated area.
Routing of Cables and Hoses, etc.
All welding electrical leads and compressor hoses shall be suspended thru the
safest possible means (not tied up with bare metal or sharp wire) from the ground
or working platform. If it is not practical to route leads or hoses overhead of a
workitraffic area, leads and hoses will be routed under the platform to eliminate
any tripping hazards, and shall always be kept out of water.T

64 Fire Prevention
Prior to commencement of any hot work activities the area that is affected shall be
free of all flammable material (combustibles: wood, paper, plastic, etc; flammable
materials: petrol, diesel, paint, thinners or mastic insulation, etc).
Fire fighting equipment such as fire extinguishers shall be readily available near
hot work activities. In addition, hot work activity inside live plants shall include
preparation of loaded water hoses prior to start activity.
At least one fully charged 6kg Dry Chemical powder fire extinguisher will be
provided immediately adjacent to the hot work location.
Employees engaged in hot work shall be trained in the operation of fire
extinguishers before being allowed to carry out any hot work. Such fire
extinguisher operation can be discussed during tool box talks.
Hot work in elevated areas shall have a fire retardant blankets strategically placed
to ensure the capture of all sparks and/or hot motal that may be produced by the
hot work,
Hot material or sparks shall not be allowed to fall or be carried into any other work.

7. GAS CUTTING
7A Inspection of Equipment
All equipment associated with gas cutting operations shall be subject to inspection
prior to use on the worksite. Items such as hoses, gauges, regulators, torches.
and flash-back arrestors will be inspected before use is permitted and must be in
good and serviceable condition. Inspections of such equipment shall be carried
out by HSE and Construction teams. Hose connections shall be of the crimped
type, clamps shall never be allowed.
72 Use of Flash Back Arrestors
Torches and regulators require installing Flashback Arrestors in the torch and on
the regulator of both the oxygen and fuel gas cylinders. By setting the correct gas
pressure and using the right equipment and gas supply, the occurrence of
flashbacks can be minimized.
Flashback arrestors are important safety devices which are required to be fitted to
the torch and regulator to protect both the operator and equipment in cutting and

NOTE: Prevention of flashback starts by selecting the size cylinder or number of
cylinders of acetylene to adequately supply the flow rate to the tip. The following
are another cause of back feeding or reverse flow.
+ Incorrect operating procedures

+ Blocked or undersized tip or nozzle
+ Faults in gas hose fittings, resulting from wear & tear, dirt, etc.

73 Ignition of Torches
Gas torches shall not be ignited by any means other than a flint gun. Cigarette
lighters, matches or hot metal will not be used as an ignition source.

7.44 Gas Cylinders
Gas cylinder valves should be closed, and the valve protection cap shall be in
place, hand-tight, when gas cylinders are not in use.
Under no circumstances should oil or grease be used to lubricate any valves or

Gas cylinders shall be stored in areas specially defined for the purpose, ic.
outdoors, away from all sources of heat and away from gangways and other
traffic hazards. Oily rags, waste and other refuse should not be allowed to
accumulate on or near stored gas oylinders. Gas cylinders should be protected
against extremes of weather and direct radiation from sun or hot equipment.
Full oxygen, acetylene and propane in storage, shall be divided by a physical
distance of 20feet (6meters) or by a concrete fire wall
Fuel gas cylinders shall be placed with valve end up, and secured from falling
whenever they are in use or stored. A cradle or similar platform closed on alll sides
should be used when gas cylinders are transported by crane.
When gas cylinders are moved by hand, they may be tilted and rolled on their
bottom rim in short distance, trolleys shall be used when handlingimoving
cylinders from one point to another. When gas cylinders are transported by
vehicle they shall be secured in vertical position. Gas cylinders shall not be
dropped, nor will they be permitted to strike violently against any other object.
\Valve-protection caps shall not be used for lifting gas cylinders from one vertical
position to another.
It is not necessary to remove the regulators and replace the valve-protection caps
when gas cylinders, (which are in use}, are moved by means of a gas cylinder
trolley, specially designed for the purpose.
Gas cylinders which are not mounted in such trolley during use shall be secured
with a suitable steadying device to prevent them from being knocked over.
Gas cylinders in use shall be kept far enough away from any welding or cutting
‘operation to prevent them from coming into contact with sparks, hot slag, hot
metal or flame.
Gas cylinders shall not be placed where they might become part of an electric
circuit. Contact with radiators, piping systems or layout tables which may be used
as ground return circuits for arc welding machines should be avoided.
Never strike an arc against a cylinder.
Gas cylinders shall not be placed under structures where valves could be
damaged by falling objects.
The numbers and markings stamped on gas cylinders should not be tampered
with,
NOTE: Gas cylinders must be pre-color coded by the suppliers in line with
international color codes. Where colors do not clearly distinguish contents, name
of contents shall be clearly marked on bottles.

Gas cylinders whether full or empty shall not be used as rollers or supports, or for
any other purpose.
The contents of a gas cylinder shall not be used for any purposes other than the
Purpose.
Gas cylinders shall only be re-filled by authorized persons or suppliers. No person
other than the gas supplier shall attempt to recharge gas cylinders.
If a valve leak develops, the gas cylinder will be removed from service, suitably

Gauges/Valves
Before an oxygen cylinder is put into service, the valve should be opened slightly
then closed immediately. This action is known as “cracking” (shifting) the valve,
and is intended to clear the valve of dust or dirt which might otherwise enter the
regulator. Valves on Acetylene/Propane or Butane gas cylinders should not be
cracked near welding work, sparks, flames or other sources of ignition. The
operator should stand on the side opposite from the outlet when cracking the
valve.
After the gas cylinder valve has been ‘cracked’ and olosed again, the regulator
should be connected to the gas cylinder and tightened with the correct size open-
end spanner, never use a pipe wrench as these will cut into the connection nuts,
(Adaptors are required to attach some acetylene regulators to the valves). Soapy
water may be used to ascertain that a tight connection exists between the
regulators, adaptors, and cylinder valves. Care should be taken to avoid getting
oil or grease on the fittings when connecting an oxygen regulator.
To avoid error when connection hoses, the oxygen regulator connections have
right-hand threads; fuel gas regulator connections have left-hand thread.
Connections should be tight to avoid leakage.
Unless special means have been provided to protect the regulators, the pressure-
adjusting screw of the regulator should be released, i.e. turned counter-clockwise
({o the left) before the gas cylinder valve is opened,
The fuel-gas cylinder valve should be opened slowly with the special key or
handle provided for that purpose. The valve should be opened and the key should
be kept in place. The pressure in the gas cylinder will then be shown by the high-
pressure or cylinder-contents gauge.
The oxygen cylinder valve should be opened slightly so that the high-pressure
gauge hand will move up slowly. The valve should never be opened suddenly,
and the operator should not stand directly in front of the gauge faces. After the
high-pressure gauge hand has stopped moving, the oxygen-cylinder valve should
be opened fully.EPr3

If the low-pressure gauge on the regulator show a continuous and steady
increase in pressure when the torch valves are closed, the gas cylinder valve
should be closed immediately and the regulator repaired,
Hoses
Hoses are manufactured in distinctive colors for ready identification. Red is
generally recognized for fuel-gas hoses, and green for oxygen hoses. Hoses color
coding shall follow the site or country color cading standards for gas hoses. Hose
connections on the regulator should be similarly marked for identification Parallel
lengths of oxygen and gas hose should be taped together to prevent tangling.
Hoses shall only be connected by means of approved fittings, crimped type only
not with clamps as this will damage the hose. Hoses shall be protected from
kinking, tangling, being stepped on, run over by trucks or otherwise damaged.
Contact of hoses with oil or grease shall be avoided to reduce deterioration of the
uber. Spare hoses shall be stored in a cool place, and away from direct
sunlight.
Hoses shall be inspected frequently for leaks, wear and lose connections. Leaks
may be detected by immersion of the hose in water under normal working
pressure or by using soapy water solution. Leaking hoses shall not be used in
site.
POST WELD HEAT TREATMENT (PWHT)
Activity Supervisor to ensure that permit to work was raised and obtained prior to
start activity.
Perimeter around the welding joint to be treated shall be cleared from other
activities. Location of work shall be barricaded and proper signs erected to
prevent unauthorized entry of personnels.
Equipments, tools and devices shall be inspected prior to use. Cables shall be
laid properly in order not to cause a tripping hazard.
Other work crew shall be advised of the PWHT activity to prevent impacts to other

44 Scope
This Safe Work Procedure for Excavation is established for the implementation
during the execution of EPF2 Project. This applies to all CONTRACTOR
employees working on the Project. It is designed to minimize risk, to prevent
accidents and waste of life through promotion of safe work practices and
behaviours during the execution of the activities for inside the BLOCK S2 area,
Republic of YEMEN
The purpose of this procedure is to ensure the safe execution of all excavations
during the construction Project. This procedure covers all excavations to be
carried out at the worksite, in any area under CONTRACTOR authority or control,
whether carried out by manual or mechanical means.

SEQUENCE OF PROCEDURE
Permit to Excavate
Internal Permit Request will be raised by the concerned manager, supervisor or
foreman who will require permit and will do excavation in specific location for
EPF2 PROJECT
Permit coordinator will prepare Excavation Permit to be signed by COMPANY
HSE Representative with attachment of JSA and the marked up drawing
specifying the exact locations.
The issuing of the PTW (see Appendix |) shall be made in compliance with the
project procedure (Permit to Work).
The Excavation Work Permit shall be submitted to the COMPANY relevant
Construction Manager or its representative for processing and approval. Clearly
marked up drawings of the proposed excavation location shall be provided by the
CONTRACTOR.ouv

COMPANY Construction Department shall maintain up to date drawings of all
underground services that are present or existing. COMPANY will review the
nature, reason and method intended to be used to carry out the excavation and
determine if any underground services may be affected by the excavation.
Where services are present, the drawing attached to the permit will be marked
up showing the location of the services and their co-ordinates.
COMPANY HSE supervisor or its representative will approve the Permit to Work
after all necessary documents and communication done with COMPANY
Construction Representative. CONTRACTOR will then be responsible for all the
phases of excavation.
COMPANY may also stipulate any special precautions that are required to
ensure the safety of personnel carrying out the excavation.

Access to and Egress from all excavations is to be constructed in a manner and
from material that will provide safe and unhindered access to and egress from
the excavation

Barricading & Edge Protection
Where applicable, a barricade placed 1 meter from each open edge on all sides
shall protect all excavations exceeding 304mm in depth. Barricades will be not
less than 300mm in height and be of solid construction. Barricades shall be
erected prior to the commencement of excavation. All excavation barricades
shall display “DANGER EXCAVATION’ signs to give warning from all directions.
Upon reinstatement of the excavation all such barricade shall be removed,
Placement and Removal of Spoil
Spoil removed from an excavation shall not be left in close proximity to the
excavation wall. A minimum distance of 1 meter from the edge of the excavation
shall be maintained. Spoil should never be a distance close to the edge less than
the depth of the excavation, e.g. with an excavation five meters deep the spoil
shall be no less than five meters from the edge.
Protective systems depth of a trench or excavations of five (5) feet or more shall
be protected by either sloping or battering of the sides by the use of excavator.
When sloping and battering cannot be employed as protection; benching method
shall be applied.
Soil Type Classification
The Classification of Soil (as per OSHA) is for the purpose of Sloping and
Benching excavation walls as protection from collapsing excavation walls to
prevent injuries to employees working inside excavation as per OSHA standard,
Stable Rock
Means natural solid mineral matter that can be excavated with vertical sides and
remain intact while exposed.
Type A
Means cohesive soils with an unconfined, compressive strength of 1.5 ton per
square foot. Examples of cohesive soils are: clay, silty clay, sandy clay, clay
loam and, in some cases, silty clay loam and sandy clay loam
Type B
Means cohesive soils with an unconfined compressive strength of greater than
0.5 ton per square foot but less than 1.6 ton per square foot. Examples include
angular gravel (similar to crushed rock), sit, silt loam, sandy loam,
Type C
Means cohesive soil with an unconfined compressive strength of 0.5 ton per
square foot or less. Examples include Granular soils including gravel, sand, and

C. BENCHING. There are two basic types of benching, simple and multiple. The
type of soil determines the horizontal to vertical ratio of the benched side.
As a general rule, the bottom vertical height of the trench must not exceed 4 ft
(1.2 m) for the first bench. Subsequent benches may be up to a maximum of 5 ft
(1.5 m) vertical in Type A soil and 4 ft (1.2 m) in Type B soll to a total trench
depth of 20 ft (6.0 m). All subsequent benches must be below the maximum
allowable slope for that soil type. For Type B soil the trench excavation is

Where practicable, spoil will be removed from the excavation area on a daily
basis.
The CONTRACTOR shall be responsible for reinstatement of any excavation.
Equipment and Materials
All heavy equipment (compressors, generators, etc) shall be kept at a minimum
distance of two meters (or the depth of the excavation) from the edge of the
excavation. Wheel locks are to be engaged on any wheeled vehicles and
equipment.
Fixed or semi-fixed equipment (pipe supports, pads, cables otc.) in close
proximity to the edge of an excavation shall be relocated or supported to prevent
their collapse into the excavation
Excavators may only be utilized as lifting equipment if the lift is directly involved
in the excavation (movement of barricade equipment, large rocks etc.). The
placement of pre-cast pits, chambers, pipes or any other equipment must be
Undertaken using properly certified lifting equipment.
During mechanical excavation an exclusion zone will be established around the
machine encompassing its full operational radius. No personnel may enter this
zone during the machines operation. Where spotters are to be utilized during
excavation, the spotter must be positioned outside the operational radius of the
machine,
Road Crossings and Closures
Road closures must have prior approval from COMPANY.
Where it is necessary to clase road when the excavations will pass through a
road crossings; temporary diversions shall be provided along with directional
signs. All road closures must be approved by COMPANY prior to close any

Temporary walkways or bridges shall be provided in areas of normal pedestrian
traffic and such walkways or bridges shall be properly erected, inspected and
tagged as temporary platform.
69 Archaeological item discovered during excavation work
In any cases an archaeological items found, the following shall apply
Objects found during the works shall be notified immediately to COMPANY.
If item is found, the following steps are followed:
+ Work in the vicinity of late findings is temporary stopped.
+ CONTRACTOR Reports to COMPANY Site Construction Manager (Formal
Communication),
+ If the findings are not significant based on the evaluation of COMPANY.
Assessment and proper documentation will be made with all protection as
deem necessary. When the work will be resumed.
+ Ifthe finding is significant, COMPANY will notify Yemeni Authority to perform
further evaluation to determine the mitigation measures and/or excavation
The mitigation measures will be implemented prior to resuming the
excavation works.
