QUALIFICATION
 The search is open to all residents of Barangay Tabang, Guiguinto,Bulacan
 Academic Excellence Awardee must be:
- a graduate of SHS with Highest Honor for the S.Y 2018-2019
- a Degree holder with Latin Honor S.Y 2018-2019
- a Board/Bar/Civil Service Passer for the year 2018-2019
 Extra-Curricular Awardee should have been a Regional/National/International
representative of any extra-curricular activities.
GUIDELINES
 Lack or non-compliance of search requirements will result in automatic
disqualification
 All decisions of the SK are final. No correspondence pertaining to these
decisions shall be entertained.
 When necessary, the SK of Tabang will request for additional data.

REQUIREMENTS
The applicant shall submit two(2) copies of the following documents:
 Accomplished Nomination Form
 Proof of Residency
 Additional information Academic Achievement Profile (If any)
 Additional Information for Extra-curricular Activities Profile (If any)
 Supporting Documents
