GUITNANGBAYAN ELEMENTARY SCHOOL

FACILITATOR

- oversees the overall progress of each project in the school


- serves as coach and provide technical assistant to every team
- monitors and evaluate the project
- calls for meeting /conference with the team leaders and
member whenever necessary
- motivates, supports and helps maintain good camaraderie,
teamwork and open communication

TEAM LEADER

helps the member to do their roles and responsibilities


- leads the team in brainstorming to come up with a decision
that concerns the team and the process of the project
- sets meetings/conference with the team members,
facilitators, coach and School Head whenever necessary
- motivates the team and maintain teamwork
- consults/seeks assistance from the Facilitators, coach,
School head and other person who are familiar with the CI
processes regarding the steps and team concerns
- assigns task to every member about the data gathering
- collects the data gathered by the team and update the CI template
- plans activities based on the data gathered
- updates the team about the progress of the project
- reports the progress of the progress to the facilitators, coach and
School head
- participates actively in brainstorming and other activities
concerning the project

DOCUMENTATION

- keeps important files of the project from


- organizes the data collection plan
- monitors the data collected and make necessary suggestion
- documents all important events/activities regarding the project
- updates the team leader about the data collected
- provides the facilitators, team leaders and members of the
updated record of the data gathered
- provides assistance, ideas in the brainstorming
- participates actively in brainstorming and other activities
concerning the project

SCRIBE

- records the minutes of every meeting done by the team


- keeps the MOMs available, neat and organize for further reference
- assists the team leader in meetings by providing information
based on the last meeting
- reminds the team leaders and members about the agreement and
actions that needs to be done based on the MOMs
- provides assistance, ideas in the brainstorming
- participates actively in brainstorming and other activities
concerning the project
COMMUNICATION

- makes plan on how to communicate the project to stakeholders


(Communication Plan)
- assists the team leader in information dissemination thru making
memo/communication about meetings and conferences about the
projects
- provides assistance, ideas in the brainstorming
- scouts for a possible stakeholders that will support the project
- makes necessary letters, communication (solicitation, programs,
etc.) in connection with the project
- maintains open communication among the team by reminding
them of the different activities and plans of the project
- participates actively in brainstorming and other activities
concerning the project
- promotes the school’s programs and projects to strengthen
linkages

PROCESS OBSERVER
-
- provides assistance, ideas in the brainstorming
- records observation of the different activities, events (meetings,
orientation, etc.) done by the team
- provides assistance to the team leader/member by making
suggestions based on the observation
- observes the attitudes, reactions of the team during the meetings
and other activities and talk it over with the team leader/member
- assists team leader in data gathering and other assigned task
- opens to the teams reaction on the observed behavior and raised
suggestions
- participates actively in brainstorming and other activities
concerning the project
- gives more focus on the different details of activities undertaken

Members - participates actively in brainstorming and other activities

- provides help and assistance in lieu of any of the above
mentioned roles
- fosters deep sense of willingness during the implementation of
the project
