I, AUGINE R. BUMANGLAG, Filipino citizen and resident of Brgy.

accordance with law, do hereby depose and states, - That:

1. This testimony was taken and transcribed by ATTY ERNIELINEO B.

LABRADOR inside his office at LABRADOR DAGDAGAN MARCOS
LAW OFFICE located at Paoay, Ilocos Norte;

2. I am answering each and every question propounded to me to the best of

my own and personal knowledge and based solely on genuine facts and
circumstances fully conscious that I am under oath and that I may face
criminal liability for perjury or false testimony;

3. Hereunder are the questions propounded to me by Atty. Labrador and I

answer them as follows:

Q4: What is your present position at the Regional Trial Court Batac, sir?
A4: I am the Court Social Worker, sir.

Q5: What is your participation in this petition for adoption, sir?

A5: I was the one who conducted the Pre-Adoption Counseling service to the
Spouses BIENVENIDO B. MANEGDEG, MARIETTA M. ORCINO-
MANEGDEG and also to Ms. MARGIE PAYAS PEDRO, sir.

Q6: I am showing you the Certification issued by the Office of the clerk of
Court and signed by you, can you please go over it and kindly tell this
Honorable Court if this is the same affidavit you are referring to?
A6: Yes, Sir. It is the same.

Court and signed by the Court Social Worker, the original copy of which is
marked and attached hereto as Annex/Exhibit “Q- and Q-1”, of the
Petitioners’ evidence.)
