SUBJECT: Designation of Undersecretaries and Assistant Secretaries as Team
Leaders of the DOH Functional Management Teams for the
Implementation of the FOURmula One Plus for Health Strategic
Framework
In view of Department Order No. 2018 - 0301 entitled “DOH Functional Management
Teams for the Implementation of the FOURmula One Plus for Health Strategic Framework”, DOH
officials are hereby assigned to various teams for the attainment of the goals and objectives of
FOURmula One Plus for Health Strategic Framework (F1+ for Health).
To ensure efficiency and accountability among DOH units, the designation of DOH officials
shall be guided by the following:

1. The Undersecretaries and Assistant Secretaries shall be designated as Team Leader and
Assistant Team Leader, respectively;
2. The Team Leader and Assistant Team Leader shall each be responsible for the attainment
of the performance targets of the DOH units under their team;
3. The Team Leader and Assistant Team Leader shall agree amongst themselves their focus
of assignments and accountabilities;
4. The Team Leader shall exercise overall supervision with focus on strategic directions and
decisions;
5. The Assistant Team Leader shall provide support to the team leader and focus on ensuring
operational directions and decisions;
6. The Directors, chiefs of division and program managers shall compose the immediate
members of the team to ensure complete staff work and implementation of the team
initiatives and interventions.

A. Office of the Chief of Staff (OCS) and Health Regulation Team (HRT)

Undersecretary Rolando Enrique D. Domingo shall lead in the oversight of the OCS and the
HRT, assisted and supported by Assistant Secretary Charade B. Mercado—Grande.

B. Health Policy and Systems Development Team (HPSDT)

Undersecretary Mario C. Villaverde shall lead in the oversight of the HPSDT, assisted and
supported by Assistant Secretary Lyndon L. Lee Say.

C. Public Health Services Team (PHST)

Dr. Myrna C. Cabotaje, Director IV is hereby designated as Oflicer-in-Charge

Undersecretary and shall lead in the oversight of the PHST, assisted and supported by Assistant
Secretary Maria Rosario S. Vergeire.

D. Health Facilities and Infrastructure Development Team (HFIDT)

Undersecretary Lilibeth C. David shall lead in the oversight of the HFIDT, assisted and
supported by Assistant Secretary Nestor F. Santiago, Jr.

E. Field Implementation and Coordination Team (FICT)

Undersecretary Gerardo V. Bayugo shall lead in the oversight of the FICT, assisted and
supported by the following Assistant Team Leaders:
1. Assistant Secretary Maria Francia M Laxamana for Luzon
2. Assistant Secretary Elmer G. Punzalan for NCR and Metro Manila Hospitals
3. Assistant Secretary Abdullah B. Dumama Jr. for Visayas and Mindanao

F. Procurement and Supply Chain Management Team (PSCMT)

Undersecretary Ma. Carolina Vidal-Taifio shall lead in the oversight of the PSCMT.

G. Administration and Financial Management Team (AFMT)

Undersecretary Roger Tong-an shall lead in the oversight of AFMT.

Representation of the DOH to high-level boards and inter-agency committees are hereby re-
assigned to the Undersecretaries and Assistant Secretaries in accordance to team mandates, as
provided in Annex A.

Under this Order, the abovementioned Team Leaders and Assistant Team leaders shall act on
all matters pertaining to the designation consistent with the functional structure for the
implementation of the Fl+ for Health, as provided in D0 No. 2018- 090! .

All issuance inconsistent or contrary to the provisions of this Order are hereby revised,
modified, repealed or rescinded accordingly.

This Order shall take effect on 3 September 2018. ‘
