This form serves as an authorization for City Garden Grand Hotel to debit the credit card number listed below for all charges indicated.

The hotel must receive authorization at least 24 hours prior to the expected arrival. Due to the merchant bank terms and conditions,

authorization forms must be attached, with copies of both sides of the credit card and the cardholder’s photo identification.
If airport transfers are needed, kindly specify car type:

TERMS AND CONDITIONS

• The HOTEL’s standard check-in time starts at 2:00 p.m. on the day of the arrival. Check-out time is at 12:00 noon.

• We require P 2,000.00 incidental deposit per room, amount is refundable. All mini bar consumptions and other charges shall be

deducted from the incidental deposit. All incidental expenses incurred by the guests shall be settled in full upon check out.

• Cancellations made Three (3) days prior to arrival is subject to 100% of total room charges.

All No Show are subject to a fee, which is equivalent to 100% of total room charges.

• Additional room accommodation may be allowed but is subject to room availability.

Reduction on the guaranteed number of rooms will be charged based on the HOTEL’s cancellation policy.

PAYMENT TERMS

All bookings are on PERSONAL ACCOUNT, all room bookings will be confirmed only if guaranteed by any of the following:

• Cash deposit;

• Credit Card Payment or; (Pre-authorization of Credit card is required upon check-in)
• Check Payment – required seven (7) working days before the arrival date. Personal checks will not be honored.
