LIST OF REQUIREMENTS FOR ACCREDITATION (AFTER REGISTRATION TO CDA)

FOR FRANCHISE HOLDERS ONLY:

1. Letter of Request signed by the cooperative’s Chairperson

2. Photocopy of Certificate of Registration issued by the Cooperative

Development Authority (CDA);

3. If the units are owned by individual member-operators, OR/CR of at least

15 units (for tricycles, PUJs, Aircon Vans, Taxis or Multi-cabs)

4. Management Agreements between the cooperative and the member-

operators (Pro-forma provided);

5. Copy of franchise or Certificate of Convenience.

6. For tricycles, favorable endorsement from the concerned LGU or TRU;

7. Bank Certification of Deposit representing the paid-up capital of the

cooperative that will operate the following:

• PUJs and Multi-cabs – Php 300,000

• Airconditioned Vans, Taxis, or TNVS – Php 750,000
• PUBs and Minibus – Php 1 Million
• Trucks for Hire (2 units) – Php 200,000
• Tricycles- Php 15,000

8. Affidavit that there was no payment made for any transaction with OTC and
acknowledging that approval for TC accreditation is not an assurance that
CPC/franchise will be granted (Pro-forma provided)
