The following constitutes the agreement between BANAHAW HEALS SPA with postal address at # 379
Unit 2, Dr. H Serafica, Real St., Ormoc City Leyte, and Veronica P. Lopez hereinafter referred to as the
FIRST PARTY OWNER.
And,
GENERAL RESPONSIBILITIES
1) DUTIES TOWARD THE PROFESSION – The MASSAGE THERAPIST shall strive for excellence and
advance the moral reputation of the profession by conducting himself/herself with honor and
integrity.
A) A MASSAGE THERAPIST shall uphold the dignity and integrity of the profession shall not
directly or indirectly participate in any immoral or illegal practices, and shall report to the
proper authorities any colleague or stakeholder who engages in such practices.
B) A MASSAGE THERAPIST shall foster a broader cultural outlook and ever increasing interest in
research and innovations that would contribute to the growth and advancement of the
profession.
2) DUTIES TOWARD EMPLOYERS – The MASSAGE THERAPIST shall be fair, honest, and loyal in
dealings with employers.
A) MASSAGE THERAPIST should be committed and shall comply to at least six (6) months with
one (1) year service in the company. Failure to exceed the number of months required for
work may be subject to a penalty fee of ₱ 10,000.00 that has to be paid to the owner.
B) A MASSAGE THERAPIST shall not enter into any agreement with any institution or enterprise
of suspicious or ill repute or which is in any manner involved in illegal activities, or whose
terms of agreement violate any provision in this code.
C) A MASSAGE THERAPIST shall avoid any conflicts of interest, and shall not engage in any
activity that competes with or any way jeopardizes the legitimate goals of the wellness
Enterprise or Training Institution which the therapist is bound by mutual agreement to serve.
D) A MASSAGE THERAPIST must wear their proper uniforms inside the company. Wearing
uniform outside the company is prohibited.
E) A MASSAGE THERAPIST shall not take any alcoholic drinks and cigarettes during work time.
F) A MASSAGE THERAPIST shall maintain cleanliness and orderliness inside the Company,
observe silence, and follow the rules and regulations of the Company.
G) A MASSAGE THERAPIST shall inform the personnel about leave and vacations one (1) week
prior to date.
H) A MASSAGE THERAPIST shall render an eight (8) to ten (10) hours of work per day and report
for work six (6) days per week.
A) A MASSAGE THERAPIST shall attend to clients faithfully and conscientiously, securing for each
one the benefits obtained from professional skill and care, without imposing terms and
conditions which would diminish the quality of client care.
B) A MASSAGE THERAPIST shall observe professional secrecy and “delicadeza” in all private and
confidential matters personal to the client, including ideals, beliefs, values, customs and the
like except in the interest of justice, public health or public safety.
A) The company shall provide the training upon signing of contract, with ₱100.00 allowances
daily and commission in all services done. Cancellation and/or breaking of contract before 1
year means that A MASSGAE THERAPIST must pay ₱10,000.00 for the training and as a Penalty
Fee. Certificate of Employment is not applicable.
Employer reserves the right to terminate or cancel this contract after observing due process for
just or valid causes. Termination may be due to any but not necessarily limited to the following;
Subject to existing labor laws the Massage Therapist/ Employees shall be covered by SSS and
the payment of the premiums, PhilHealth dues of which shall be shared by the employee and
the employer in proportion to that mandated by the laws on the matter.
It is hereby acknowledge that both parties have entered this agreement on his/her/their own free will,
with complete knowledge and understanding of his obligations, duties and responsibilities as stated
within this contract.
