To whom so ever it may concern

This is to certify that Mr. Babu aged 63 (M) residing at Renjith

Bhavan,Thalikuzhy, Kallara P.O is suffering with Peripheral Vascular
disease and IHD. He is unable to walk even short distance and needs
physical support for all his daily needs and activities.

His wife Mrs.Syamala aged 57 (F) residing with him is also a known
diabetic and Hypertensive and is also suffering from Chronic Backache
due to Disc Prolapse.

Please be noted that they are both under my treatment since last 7
years.
