AFFIDAVIT OF LOSS
I, EDWIN P. TAMAYO, Filipino, of legal age, married and a resident of
Bagumbayan, Tuguegarao City, after having been duly sworn in accordance with
law, hereby depose and say:

1. That I graduated from Saint Louis Colleges of Tuguegarao (now USLT) with a
degree, Bachelor of Science in Commerce Major in Accounting in the year 1984;

2. That as such, I was a bona fide holder of a DIPLOMA issued to me by the

said educational institution;

3. That sometime in 1992, I transferred my residency from Carig to

Bagumbayan and as such I likewise transferred all my personal belonging to my new
residence;

4. That I might have inadvertently misplaced my diploma when I moved my

my personal belongings at my house at Bagumbayan;

5. That I tried and exercised all diligent efforts in order to find and recover the
said diploma but all of my efforts were of no avail;

6. That I sincerely believe that the said diploma is now beyond recovery

7. That I am executing this affidavit to attest to the truth of the foregoing facts
and for whatever legal purposes it may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 9th day

of March in Tuguegarao City, Philippines.
